JOB SUMMARY: The On Call Nurse will be the first line for phone calls coming in from patients and families or field staff. The On Call Nurse will be able to answer questions regarding hospice services, re-fill prescriptions, assist with ordering supplies and DME. Must be available while on call to see patients in their residence to include, but not limited to homes, facilities, and hospitals. Must be available to perform admissions and attend discharges within scope of practice.
ESSENTIAL JOB FUNCTIONS (As deemed by the Board of Nursing within the State in which you are practicing):
Will perform after hours and weekend visits and admissions as needed.
Follow up on phone calls by either directing the phone call to the DPCS, RN Clinical Supervisor/Manager or RN Case Manager and/or taking care of the need immediately per scope of practice
Dispatch field nurses and other appropriate staff to patient's residence in a timely manner
Re-fill prescriptions, order supplies and DME, and assist in any other coordination needed by the patients and families
Document calls, conversations and outcomes of call-in software system
Receive start-of-shift report and give end-of-shift report in accordance with the location workflow
Will perform any other job duties as assigned.
Qualifications
REQUIRED EDUCATION & EXPERIENCE:
Must be at least 18 years of age
Must be a Registered Nurse or LPN/LVN with knowledge of hospice, telephone triage preferred
Must have an active RN or LPN/LVN license in good standing with the state
REQUIRED KNOWLEDGE, SKILLS & ABILITIES:
Must show aptitude of computer data entry and the use of current software systems, such as word processing, spreadsheets and projects
Must possess the ability to deal with stressful situations, as well as grieving families
Must be able to take patient/family phone calls to determine needs of the caller
Must go out to visit patients according to patient needs and Bristol Policy
Must demonstrate the ability to provide professional and courteous interactions with patients, their families and staff under sensitive situations
OTHER REQUIREMENTS:
Willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
Must be dependable and flexible in work hours
WORK ENVIRONMENT:
Must comply with company policies, procedures and processes in addition to Hospice and Homecare rules and regulations
Must comply with harassment and discrimination policies
Must foster good working relationships
Must be able to speak and write in English
Must be a licensed driver with licensed automobile that is insured in accordance with organizations/state requirements and is in good working order
Must be willing to submit to a criminal background check as well as a drug/alcohol screening upon hire and periodically thereafter
PHYSICAL ENVIRONMENT:
Must be able to see and hear with/without accommodations
Requires consistent wrist movement
Requires sitting for prolonged periods
Must be able to bend, stoop, kneel and climb
Must be able to lift up to 20 lbs. without assistance and up to 50lbs. with assistance
Must be able to communicate with patients' families and other health care workers
Speech clarity and speech recognition are vital abilities for performing the job
