The Office Nurse, under the supervision of the Practice Manager, is responsible for assisting in the delivery of health care and patient care management.


DUTIES AND RESPONSIBILITIES:

#Triage patient phone calls to facilitate appropriate level of care, coordinate office visits.

#Triage sick walk-in patients for level of urgency, initiate diagnostic tests.

#Review complicated test results with patients at provider#s request.

#Prepare and maintain clinical supplies and equipment related to independent care management.

#Maintain medication records, administer medication upon physician order, and phone in of pharmacy orders as requested.#

#Vaccine administration support (pediatric, patient overflow):#

#Conduct patient education visits (e.g. diabetes, COPD, Asthma self-management teaching):#

#Assist Providers with nurse related responsibilities: vaccinations, follow up with labs, triage patient, obtain more details.

#Injectable medication flex pen teaching, f/u with patient#s compliance with BGT/insulin, provide more training, if necessary, work closely with VNA nurse to continue monitoring non-compliant patients.#

#Perform Annual Wellness Visits

#Processing Prior Authorizations for prescriptions as assigned

#Perform in house tests, including Urinalysis, Blood Glucose Test, INR, Hemoglobin A1c, etc.#

#Prior authorizations for medications not approved.

#Refilling prescriptions

#Patient education on diet, diabetes management, cholesterol, asthma and as requested by providers.

#Performing Immunizations and Antibiotic Injections, administering medications, nebulizer treatments

#Perform and assist in wound care as directed by the provider.

#Additional tasks as assigned. EDUCATION AND RELATED EXPERIENCE: Required:

#Education/training: Nursing degree (Associates or Bachelor of Science in Nursing),#

#Massachusetts State Licensure as LPN or RN.

#Knowledge of medical practice and care

#Knowledge of examination, diagnostic and treatment room procedures

#Knowledge of medical equipment and instruments

#Knowledge of common safety hazards and precautions

#Knowledge of pharmacology as demonstrated by completion of appropriate CME or Lawrence General Hospital nursing orientation requirements

#Ability to assist in a variety of treatments and medications

#Skill in developing and maintaining clinical quality assurance

#Skill in identifying and resolving problems

#Ability to maintain records and record test results

#Ability to interpret, adapt and apply guidelines and procedures

#Ability to use good reasoning and judgment and react calmly in emergency situations

#Ability to establish and maintain effective working relationships with patients, medical staff, coworkers and the public

#Ability to utilize effective communication tools

#Proficient computer skills and knowledge of MS Office programs

#Flexibility and willingness to deal effectively with interruptions throughout the working day

#Ability to work independently yet functions as part of a team.#

#Ability to organize and prioritize workflow independently

#Excellent verbal/written communication skills Preferred:

#Bilingual (English # Spanish).#
