Summary:
Job Responsibilities:
Completes timely, thorough assessment reports with problem-related treatment plan. Completes and updates interventions/reports in timely manner for MDT Tx Plans.
Completes group graphics on time. Enters concise progress notes in timely manner related to specific tx goals. Completes discharge summary on time with specific recommendations.
Gathers all necessary info. to assess functional status (ADL's, IADL's, leisure, daily structure, social skills, etc.). Successfully administers standardized testing. Comprehensively identifies strengths/assets.
Clearly formulates clinical observations, problem-based tx plan in accordance with assessment findings; clearly communicates results to MDT. Effectively collaborates with patient, other practitioners to establish overall tx goals Reports suspected abuse cases to appropriate state agency per regulations.
Establishes therapeutic relationship with patients (rapport, boundaries, etc.). Applies treatments in accordance with treatment plan, including precautions, contraindications. Appropriately manages pt. behaviors, related symptoms.
Independently performs procedures and manages complex cases. Provides therapeutic individual and group tx utilizing established OT, psychosocial modalities. Reassesses/reviews pt. tx and progress and consults/collaborates with MDT in timely manner in order to facilitate tx plan updates. Collaborates with MDT to develop discharge plan and related follow-ups.
Thoroughly orients students, new hires in accordance with program standards per supervisor assignment. Supervises COTA, students in accordance with program standards.
