JOB DESCRIPTION AND RESPONSIBILITIES:

Performs comprehensive evaluations, develops, and appropriately progresses individualized treatment plans, provides thorough education to patients and caregivers in a manner they can understand and plans appropriately for discharge when medical necessity no longer indicates skilled therapeutic care. Participates in departmental initiatives and demonstrates effective communication with all rehab team members to facilitate professional growth and development and overall improvement of departmental performance. Performs other professional duties associated with the care of the patient, development of discipline specific therapy services and design of program delivery.

  1. Follows all facility guidelines outlining standards of personal appearance, attendance and punctuality.

  2. Based on the Competency Program established for direct care providers, possesses the skills and competencies necessary to perform job function in accordance with population served criteria.

  3. Provides clinical and service excellence with the provision of patient care. Through their actions, consistently demonstrates best practice while communicating with and performing initial evaluations, treatment planning, treatment progression, ongoing assessments, goal setting, HEP, and discharge planning for the patients assigned to their caseload. Through comprehensive history taking and utilization of appropriate objective measures and standardized tests is able to establish patient care plans that address patients' specific clinical and functional goals. Is able to understand and comply with diagnosis specific medical instructions, restrictions and precautions. Demonstrates ability to utilize appropriate clinical decision making to maximize patient outcomes. Identifies patient barriers, such as accessibility, language, age specific needs, culture and learning differences that may impede functional progress, follows established procedures and works with rehab team to collectively resolve issues.

  4. Completes medical record documentation in a clear, accurate and timely manner that is in compliant with all state, federal and intermediary guidelines and in accordance with policies and procedures. In accordance with the medical record audit tool, demonstrates consistent compliance with documentation guidelines inclusive of justification of skilled therapeutic intervention, functional goal writing, and progress updates. In accordance with departmental specific procedures, appropriately documents in the medical record, any patient safety related matters identified and addressed during therapy. Understands and utilizes clinical coding appropriately.

  5. Effectively performs caseload management to maximize patient outcomes. Carries a full caseload, ensures management of patient schedule, including communication with appropriate staff and management regarding openings and caseload concerns. Utilizes support staff and assistant therapists in an appropriate manner to assist with schedule management and patient care delivery. Follows all established departmental procedures in order to minimize delay in continuum of care.

  6. Understands and follows departmental operational procedures. Independently seeks guidance regarding processes. Demonstrates ability to communicate with supervisor and senior staff to facilitate problem solving strategies regarding departmental operational issues.

  7. Identifies and initiates own professional growth and development through self-study, seeking peer mentorship and formal continuing education that is relevant to the strategic objectives of the organization and their professional development plan. Advocates for their profession through participation in professional service including membership in professional associations.

  8. Demonstrates the development of skills towards providing clinical supervision and mentorship in a manner that promotes a positive learning environment and continued growth. Participates in student program by providing direct supervision or assisting in the education of departmental students. Offers timely, constructive feedback both formal and informal to students and communicates with student coordinator and school. Provides peer education through formal and informal didactic opportunities, such as in-services, hands on training, and team collaboration.

  9. Actively participates in marketing and community outreach initiatives. Represents the department and or company at community events, patient advocacy initiatives, fund-raising activities and health fairs. Performs appropriate outreach to current and prospective patients as well as internal and external referral sources.

  10. Demonstrates a commitment to performance improvement and evidence-based practice through active participation in appropriate departmental and campus wide PI initiatives. Ensures patient compliance with the various satisfaction surveys. Appropriately utilizes and records patient reported outcome surveys, compliant with departmental expectations. Completes employee surveys and works with team to identify and implement action plans. Participates in research activities as available.

  11. Adheres to discipline specific State Practice Act and other regulatory agencies. Maintains professional licensure in good standing and completes all state mandated continuing education requirements. Performs in an ethical manner in accordance with discipline specific professional Code of Ethics. Complies with all departmental and organizational regulations and educational requirements.

  12. Performs other duties as requested.


QUALIFICATIONS:

  • Current state licensure or eligibility for state licensure as an Occupational Therapist required.

  • Certified American Heart Association (AHA) BLS or completion in first 90 days of employment or as required by state regulations.

COMPENSATION:

$90/hour
