We are currently offering full-time, part-time, and per diem Occupational Therapy opportunities for our home health patients in San Mateo and Santa Clara counties. We offer flexible hours from Monday through Friday with a competitive salary rate, excellent benefits, incentive bonus, and mileage compensation.
Key Results Areas and Position Accountabilities/Competencies
- Initiates assessment based on the patients presenting problems/needs in accordance with agency standards.
- Synthesizes pertinent initial and ongoing patient data to identify speech therapy priorities for care.
- Facilitates patient’s transition to the next level of care.
- Validates patient/family learning.
- Documents all clinical activities in accordance with agency policy.
- Executes therapeutic and/or supportive interventions.
- Evaluates the patient/family response to and effectiveness of interventions.
- Accepts Case Management responsibilities when designated.
- Completes OASIS assessment per agency and regulatory policies.
- Evaluates the patient/family progress toward established outcomes.
- Follows policies related to MD orders and changes to the Plan of Care.
- Maintains required competencies.
- Meets the agency’s productivity standards.
- Computer competency demonstrated through timeliness and accuracy of documentation.
- Maintains and cleans all equipment, in accordance with agency policy.
- Participates in in-service training.
- Performs other duties as assigned.
Requirements
Requirements/specifications (Knowledge, Skills, Abilities)
- Graduation from an accredited School of Occupational Therapy and current Occupational Therapy registration in good standing from National Board for Certification in Occupational Therapy (NBOT)
- An ability and willingness to demonstrate and maintain clinical competency as required in-home health care.
- CPR certification.
- Excellent verbal and written skills.
- An ability and willingness to work with patients of various ages and needs.
- An ability to manage a sometimes chaotic work environment related to changing patient needs including work with patients with acute, chronic and complex processes.
- A positive attitude toward health care team members and diverse patient populations.
- Must be a licensed driver with an automobile that is insured in accordance with Rehab At Home requirements.
- Must have a telephone to receive, and place calls for agency operations.
- Computer keyboard skills.
