This position will serve the Portland West Side.

The Occupational Therapist provides therapy services in the patient's place of residence. Care is provided under a physician's order, and coordinated through quality practice, case management, and all aspects of the OT process. The Occupational Therapist is an integral member of the interdisciplinary team, and provides supervision of services provided by an Occupational Therapy Assistant when utilized.

Job Responsibilities / Essential Tasks:

  • Establish treatment plan appropriate to physician orders, patient evaluation findings and functional level, and within the professional scope of practice and individual level of expertise

  • Notifies the Physician of changes in patient status and initiates appropriate care plan changes

  • Evaluate home environment for hazards or barriers to more independent living. Identifies equipment needs and participates in the development of the total care plan

  • Practices with safe and up to date skill level, and solid knowledge base; including participation in in-service programs and continuing education opportunities

  • Prepares/completes clinical and progress notes, and documentation of coordination of care within timelines established by regulatory and agency standards

  • Counsel and teach patient and family/caregivers in meeting individual treatment goals, and treatment plan implementation

  • Supervises COTA's when utilized by performing and documenting supervisory visits. Follows up on supervision with clinical management team to establish that care is safe and appropriate to the patient's needs and plan of care, to ensure that relationships and goals are met

  • Willingness to help team members as needed; including participation in training and orientation of agency team members

  • Participates in quality assurance and quality improvement within the agency.

  • Performs other duties as assigned by clinical management team.

  • Maintains excellent attendance and punctuality.

The qualified candidate will have a valid OT license in the State of Oregon. Strong computer skills are necessary, as all documentation is done electronically. Experience working in Home Health as an OT or COTA is preferred, but not mandatory. You will receive a laptop, MiFi device, and mileage reimbursement as a Home Health OT.
