Occupational Therapist - SNF

Key Responsibilities

• Evaluate patients and develop personalized treatment plans based on medical diagnoses
• Implement evidence-based interventions to improve residents' functional independence
• Assess and document patient progress through detailed reports and daily notes

• Clinically supervise occupational therapy assistants
• Participate in care plan meetings and documentation

Requirements

• Graduate degree from an accredited Occupational Therapy Program
• Current New York State OT License
• Experience in skilled nursing or geriatric care (preferred)
• Strong communication and interpersonal skills
• Proficiency in electronic health record systems
