Job Description Job Description Job Title: Per Diem Home Health Occupational Therapist Company: Home Care for the 21st Century Cypress Location: Waller, Cypress, Katy, Tomball, Spring and Houston Job Type: Per Diem (1 to 4 patient visits per week) Job Description: We are currently seeking a passionate and dedicated Occupational Therapist to join our team on a per diem basis. As an Occupational Therapist, you will work with individuals to promote independence and improve their ability to perform daily activities and tasks. Responsibilities:

• Conduct comprehensive evaluations of patients' functional abilities.

• Develop individualized treatment plans to help patients achieve their goals.

• Provide hands-on therapy and therapeutic exercises.

• Educate patients and their families about adaptive techniques and equipment.

• Collaborate with other healthcare professionals to ensure comprehensive care.

• Maintain accurate and up-to-date patient records. Qualifications: - Bachelor's or Master's degree in Occupational Therapy.

• Valid state license to practice occupational therapy.

• Proven experience working with patients of all ages and backgrounds.

• Strong communication and interpersonal skills.

• Ability to work independently and as part of a team.

• Commitment to providing high-quality patient care. Schedule and Compensation:

• Per diem position with flexible scheduling.
