Position Summary:

The Nutritional Dietitian coordinates the provision of nutrition improvement with other members of the health care team. The Dietitian identifies patients at nutrition risk, assesses nutrition needs according to age and disease specific requirements and develops, implements and monitors the nutrition care plan and outcomes of intervention. Provides nutrition education to patient, families, health care professionals and the community. Supports of the nutrition care delivery system and participation in performance improvement and research activities. This role will support Urology Nutrition (75%) and executive health consultations and independent nutrition visits (25%).

Key responsibilities include, but are not limited to:

  • Develops an individualized plans based on age, culture, social consideration conditions, and other relevant needs.

  • Determines nutrition interventions based on patient's needs and expected outcomes.

  • Identifies interdisciplinary resources needed to assist in implementation and makes referrals as needed.

  • Assures all actions are based on scientific, ethical, or ecological principles; relevant policies/procedures; proven effectiveness; past experience; Medical Nutrition Therapy practice standards; patient/family requests and values; and legal parameters.

  • Evaluates, systematically and continuously, the patients' response to interventions.

  • Plans and presents educational programs which meet the assessed needs and capabilities of inpatients, outpatients, community groups, and health care professionals using appropriate department educational materials and lesson plans. Considers population served.

  • Assists patients, families and community members in identifying and securing available resources to achieve necessary goals.

  • Communicates and receives verbal and written information in a clear, concise, accurate, and timely manner.

  • Implements quality practice by following system policies/procedures, practice guidelines, and Medical Nutrition Therapy protocols.

  • Systematically evaluates quality and effectiveness of practice and revises practice to incorporate evaluation results.

To qualify for this role, you must possess:

  • College graduate with a B.S. in Nutrition and / or Dietetics or closely related field.

  • American Dietetic Association Registered or registry eligible, ADA registration within 90 days, MN State Licensure within 6 months of hire.

  • One plus year as a clinical dietitian on non-specialty practice areas. One to two years of position related experience in nutrition improvement.

  • Nutrition Care Planning Skills. Medical nutrition therapy skills. Excellent communication skills. Ability to multi-task on an ongoing basis. Good organization and prioritization skills. Continuous education in related clinical/nutrition areas.

  • Must be an Organizational Citizen, Relationship Builder, Superior Patient Advocate, Committed to being an Effective Systems User, Self-Motivated Problem Solver, Continuous Improver and Technically Advanced in Job Role.

Preferred:

  • Master's Degree in Nutrition or Dietetics or closely related field

  • Two to five years of experience as a dietitian or nutrition consultant.

  • Strong Computer and analytical skills strongly preferred.

The Community Mental Health Worker position provides community based support for individuals with mental health diagnoses enrolled in a clinic based care coordination program. Duties include promoting wellness by providing culturally appropriate health information, providing coaching to patients to assist them in achieving the goals on their care plans, and connecting patients with needed community programs and resources. Assists patients/families with setting up and preparing for appointments, accompanying patients to appointments as appropriate, and ongoing follow up with patients. The person in this position works within a team of primary care physicians, behavioral health clinicians, and social workers. Other duties Include: * Bridging the gap between communities and the health and social service systems * Navigating the health and human services system * Advocating for individual and community needs * Building Individual and Community Capacity * Demonstrates ability to provide applicable care/service adjusting approaches to reflect developmental level of population served. * Performs other duties including Performance Goals developed by manager and employee and reported in the employee's Performance Review as part of UMPhysicians' Performance Management tools To qualify for this role you must posses: * Community health worker, peer specialist, care manager assistant or similar * 3 years' experience working on community health projects, programs or initiatives. Community-based leadership experience. Experience teaching or presenting information to groups. Experience, competence and sensitivity in working with diverse communities * Fluent in English. Connected to the community they serve. Familiar with basic community health concepts and specific health issues * Strong organizational and data collection skills. Strong computer skills; proficient with Microsoft Office Suite. Documentation skills especially with health information (accurate, regular, ongoing) * Ability to listen and respond to feedback. Willingness to mentor Community Health Worker interns * Current Driver's License. * Interpersonal skills, ability to work effectively in teams, commitment to interdisciplinary care.
