Education/Experience: Experience with central venous catheter care and patient education. Ability to self-organize, pre-qualify and maximize field customer service activities and travel. Prior experience with customer service required. Flexible communication style, highly motivated and excellent listening skills required. Registered Nurse with nutrition support and/or home infusion experience preferred.
Certificates and Licenses:
Current state licensure as a registered nurse. CRNI and/or PICC Certification preferred. CPR certification required.
Duties and Responsibilities include the following. Other duties may be assigned.
Coordinates hospital discharge with case manager, hospital team and patient/family.
Schedules visits for home parenteral nutrition patients, following regulations and policy relative to licensure, state nursing requirements and compliance guidelines.
Assess and teach patients on enteral and parenteral nutrition therapy administration during initial visit and ongoing, as needed.
Communicates care plan and patient visits with home health agencies, as needed.
Participates as necessary in internal and external care conferences related to ThriveRx patients.
Provides ongoing patient monitoring and intervention according to the Nursing Standards of Care to include; telephonic follow-up, patient education, home health agency management, hospital and home evaluation, as required.
Identifies and resolves customer service issues in territory with appropriate communication with ThriveRx clinical team, management, medical team and patient.
Timely and appropriate documentation in the patient medical record, as indicated.
Keeps current with home nutrition support and central catheter care research and regulations.
Assist with development of clinical standards, policies and procedures and clinical forms.
Develops and ensures appropriate distribution of consumer/patient educational materials.
Works with Account Manager to create and implement a territory marketing plan to include continuing education presentations, physician and medical team meetings and consumer events.
Participate in Consumer and Nutrition Advisory Board projects and meetings.

Able to effectively communicate with ThriveRx team, medical team members and patients, following acceptable communication methods and consent documentation.
Maintains confidentiality of patient and competitive information.
Completes required reporting and accounting of time, expenses and other reports as required, within the stated timeframe.
