General Description:

  • Serves as a member of management team and develops effective working relationships with the director of nursing, administrator and the medical director.

  • Participates in the development of center goals

  • Assists in the development and implements procedures to meet the goals of the center.

  • Develops positive partner relations policies and environment to achieve firm, fair, consistent supervision and a department team

  • Administers policies and formulates procedures for the nursing department

  • Support and administers a departmental budget based on the nursing department's goals and projected revenues

  • Participates in establishing a competitive wage, salary, and benefit plan for nursing service staff.

  • Supports the operation of the nursing department in a cost-effective manner

  • Involved in the recruitment, selection, training, evaluation and retention for all nursing staff

  • Assists in the development and implementation of center's master staffing plan based on patient's needs, nursing service goals and standards, department budget and state law.

  • Promotes consistent assignment scheduling to meet patient care needs.

  • Formulates, implements and evaluates a department plan for orientation and partner development

  • Promotes and delegates accountability for maintaining an effective performance appraisal system for nursing

  • Promotes resolution of conflicts through delegation and accountability for positive supervision

  • Promotes and delegates personnel policy compliance with supervisors

  • Creates a work climate that promotes a positive quality of work and minimizes institution's legal risk

  • Establishes linkage with existing community resources

  • Assesses supplies and equates needs for Nursing Department and sees that units are adequately stocked and maintained.

B.Specific Items: (Performed by DON or delegated to qualified staff).

  • Participates in development and maintenance of nursing services' philosophy and objectives, standards of practice, policy and procedure manuals and job descriptions for each level of nursing personnel

  • Assists in the development of 3 year capital expenditure budget for Nursing Department

  • Assists in the development of at least biweekly (every two weeks) work schedules for Nursing Department post and maintains copy as worked.

  • Acts upon recommendations from the DON, RN Supervisors and/or supervisory LPNs regarding hiring and firing of nursing personnel; assist with recruiting as needed.

  • Delegates and monitors orientation and initial training for nursing personnel

  • Coordinates with other department heads on any patient care needs

  • Participates in all department head meetings

  • Cooperates with consultants and implements their recommendations, if determined appropriate to the nursing department

UNIT MANAGER DUTIES AND TASKS

Daily

  • Provide daily direction and leadership to your nurses and CNA's. Performance behavior opportunity. Take ownership of your UNIT.

  • Assist with the staffing and scheduling of nursing partners.

  • Verify nurse and CNA coverage, assignments, lunches, showers, etc are scheduled.

  • Lead Stand-Up; cover admissions, discharges, incidents, any info that will arm your team with tools to provide excellent care.

  • Check mailbox FREQUENTLY throughout the day for orders/results/admission and discharge info etc.

  • New Admissions:

  • Identify needs and prepare for the patients arrival.

  • Insert care plans and anything that will ensure a smooth transition/admission in chart prior to patient arrival.

  • Follow up to ensure orders noted off and entered correctly by nurse

  • AM audit new admit charts for compliance and completion

  • Run Ad ministration Exception Report and New and Modified Orders Report daily and follow up with nurse.

  • Daily rounds, in person with EVE RY patient, verify appropriate care. Inspect patient room for neatness and regulatory compliance.

  • Check corridors for equipment and proper storage.

  • Ensure nurse station is free of clutter, food/drink, personal items, supplies, linens, etc.

  • Check new orders box throughout day in IMAR to verify processed, care planned, and completed

  • Verify ATB are logged, alert charting initiated and care planned.

  • Follow u p/investigate ALL incidents. DON will assist.

  • Inspect Coumadin log for new orders and completion; follow up.

  • Participate and verify new partner training progress and completion.

  • Audit lab logs and ensure notification of M.D. and Family.

  • Audit daily Skilled Medicare notes have been made Q shift 72 hours after admission and daily thereafter.

  • Follow up on alert charting daily.

  • Ensure that Psych meds have a diagnoses and behavior monitoring in place.

  • Assess call lights daily for timeliness.

  • Ensure that weekly skin assessments are complete as scheduled.

WEEKLY

  • Audit glucometer logs

  • Med room and med carts week

  • 5 star follow-up

  • Obtaining information and attending LOC each week

  • Attending care plan meetings

  • Census and condition report

  • Audit CNA documentation notebooks and dart for compliance

  • Ensure that monthly summaries and bradens are being done as scheduled.

  • Visually inspect pressure ulcers and audit wound documentation for accuracy.

MONTHLY

  • Assist with processing pharmacy recommendations

  • Assist with completion of infection control reports and logs

  • Assist with gathering QA data

  • Other duties as assigned by DO N/ADMIN

  • Participate in preparing and conducting partner performance reviews-90 day, annuals, etc.

Coordinate and communicate the care of the resident to staff taking care of the patient and ensure all new residents have complete and accurate orders transcribed. * Audit new admit charts within 24 hours of admission. * Repeat body audit within 24 hours of admission. * Attend meetings with families; review the plan of care and concerns as requested. * Communicate conditions of patient with other departments. Ensure that any recommendations made during these risk assessments are carried through to actual care of patient. * Ensure that care plans are updated continually. * Communicate and ensure patient care all staff follow plan. * Solicit family input to the care plan of the patient * Attend interdisciplinary care plan meetings for each patient, as requested. * Coordinate problem solving sessions with staff based on these care plan meetings i.e. help to resolve complaints made by residents and/or family members. * Attend morning report daily to discuss the residents on respective unit. * Attend weekly Medicare/Utilization Review Committees. Responsible to update this committee on the progress of the resident medically. Communicate results of meeting to unit staff (i.e. RN, LPN, CNAs, etc.). * Attend monthly QA Meeting and complete reports related to quality assurance. * Attend weekly 5 Star Meetings and ensure that recommendations are carried out to the actual care of the patient. * Assign duties to LPNs and CNAs. * Ensue duties assigned are completed using appropriate standards of care * Monitor call lights to ensure they are answered timely * Monitor and maintain cleanliness of Med Room, Med cars, Supply rooms and Biohazard rooms. * Ensure monthly, weekly and daily weights, as appropriate, are recorded accurately in the record of each resident. * Ensure monitoring is being done for each patient, including accurate meal intake, elimination, fluid intake and that supplemental feedings are being given as ordered. * Update and maintain accurate CNA Assignment Sheets daily. * Ensure accurate communication of resident care needs to staff, including ancillary departments. * Assume responsibility for safe administration of medications and treatments. * Ensure that MARs and treatment sheets are completed accurately without blanks. * Supervise and evaluate work performance of staff that are responsible for the care of the residents. Complete Performance Evaluations as assigned. * Coordinate nursing activities with the activities of other departments and agencies. * Supervise the maintenance of patient records, keeping them complete and accurate, according to NHC policies, HCFA regulations and Medicare/Medicaid time frames. * Make frequent rounds to ensure that nursing care is provided according to established standards, NHC policies and state law; personally check residents in whom a change in condition has been reported; and report any changes in a patient's conditions to supervisor, physician and responsible party and document. * Participate in orientation and assist with training of all nursing staff. * Educate LPNs and CNAs on proper documentation principles, according to NHC standards * Educate LPNs and CNAs on proper Policy and Procedures. * Assist physician during rounds: * Work with a physician as a problem solver/seeker. * Prepare MD list and check for accuracy * Prepare charts and attach needed additional information (eg FSBS, labs, etc.) * Ensure accurate transcription of all MD orders * Ensure timely RP notifications of all MD visits, new orders, accidents or injuries. * Serve as a positive management role model in providing guidance to nursing staff. Ensure Stand-ups/Better Way conducted on all shifts. * Deal effectively with families and community in order to provide positive image to the public. * Display a warm and friendly attitude toward visitors, making them feel welcome and comfortable (readily greeting and directing them as needed). * Thorough knowledge of the center's fire, safety and disaster procedures. * Thorough knowledge of the center's Medicare and Medicaid guidelines * Thorough knowledge of client and corporate policies and procedures. * Working knowledge of CPCS system (Admission Assessments, Incident Reporting) * Knowledge of the center's infection control policies and procedures. * Monthly surveillance of residents with infections and precautions taken to prevent the spread of the infection to other residents. * Recommend and educate staff on appropriate procedures to use when a patient has an infection. * Monitor staff to ensure they use standard precautions during care of all residents on the unit * Ensure that Stat Box is accurately maintained. (e.g. medications listed on pharmacy list match what is in the box.) * Manage Skin Care Management per NHC Standards. * Ensure communication with the physician and other departments once a week regarding progress of wounds. * Recommend changes in treatments if no progress noted in 2 weeks and obtain appropriate orders. * Ensure general skin audits are completed weekly with appropriate care changes made based on these assessments. * Ensure completeness of report to the Director of Nursing by the end of each week * Monitor completion of Monthly Summaries. * Ensure medications are reordered in a timely manner to provide timely availability to residents * Maintain a thorough knowledge of all pharmacy regulations, especially as relates to controlled substance acquisition and refills. * Monitor the timely performance of all diagnostic studies, including labs (PT /IN R), CBC, U/ A, etc.) * Log all resulted labs into the lab book. Investigate and obtain any missing lab results. Report any lab problems to ADON or DON promptly. Be knowledgeable and competent to retrieve labs from the lab computer site.
