The Nursing Facility Survey Unit (NFSU) is staffed by healthcare professionals who have been trained and certified by the Centers for Medicare & Medicaid Services (CMS) to conduct federally mandated complaint investigations and certification surveys, to determine compliance with requirements for long-term care nursing facilities (NFs) participating in the Medicare and Medicaid programs and/or licensed in the State of Oregon.

We are seeking multiple Nursing Facility (NF) Client Care Surveyors (Client Care surveyors) to join our Nursing Facility Survey Unit Locations:

  • Salem
  • 500 Summer St. NE Salem, OR
  • Tigard
  • 10775 SW Cascade Ave. Tigard OR

What you need to qualify

Three years of experience in areas related to nursing, social work, developmental disabilities, dietetics/nutrition, or sanitation AND one of the following:

  • A Bachelor's degree in a health or human services field

  • A license as a Registered Nurse.

LPN certification and/or experience will not meet the minimum qualifications.

Preferred: Experience in areas related to Nursing, Social Work, Speech Pathology, Physical Therapy, Occupational Therapy, Abuse Investigation, Certification, or Nursing Home administration.

Essential attributes

We are looking for candidates with:

  • A minimum of three years of experience in areas related to Nursing, Social Work, Speech Pathology, Physical Therapy, Occupational Therapy, Abuse investigation, Certification, or Nursing Home administration.

  • Experience in critical thinking, exercising independent judgment and decision-making in the application of laws, rules, policies, or procedures.

  • Experience writing professional reports and communicating verbally with a variety of medical and other professionals, facility management and staff, residents, relatives, and other entities, sometimes under tense or hostile conditions.

  • Experience with computer skills using a variety of software, spreadsheet software, email management, data entry, video conferencing, instant messaging, and managing a laptop.

  • Experience and knowledge of federal and state laws, rules, and regulations related to the provision of care and services in health, health-related settings, long-term care programs or facilities.

Your duties at a glance

Aging and Disability Services

As a Nursing Facility Client Care Surveyor (NFCCS), you will:

  • Ensure the health, safety, quality of life, and quality of care for all Oregonians residing in nursing facilities (NFs).

  • Obtain and maintain federal Surveyor Minimum Qualifications Test (SMQT) certification with CMS through testing and ongoing training requirements.

  • Perform work independently and with a team in accordance with CMS, ODHS and NFSU procedures and guides; and professional standards of practice.

  • Survey the quality and adequacy of care and services received by residents of NFs for substantial compliance within mandated federal regulations and state rules within required time frames.

  • Participate in combined federal certification and state licensing surveys of NFs. Surveys may be for routine inspections, for complaint investigations, or monitoring purposes. Surveys are conducted according to mandated federal and state requirements, protocol, guidelines, and time frames. Requires immediate surveyor decision making and response for those allegations involving an immediate threat to a resident's safety and welfare.

  • Review facility plan of correction (PoC) for deficiencies and advises administrator of any issues that may need further clarification for the PoC to be acceptable per Federal (CMS) and state requirements.

  • Investigate (independently or in a team) complaints of alleged noncompliance focused on the specific area(s) of concern with an expanded resident sample as required by CMS.

  • Conduct (independently or in a team) follow-up revisit surveys to determine if facilities have corrected cited deficiencies and maintained substantial compliance as alleged.

  • Monitor all incoming NF complaints and incident reports.

  • Create new intakes and enters all complaint/incident intake information into the CMS database.

  • Contact complainant(s) to verify and confirm information received and solicit additional information to assist in determining potential noncompliance with federal and/or state requirements.

  • Collect and document all additional information obtained during interview(s) with complainant(s) in the CMS database.

  • Obtain necessary facts and relevant data to compile and prepare a concise and comprehensive intake narrative for onsite investigative activities.

  • Develop and document allegation statement(s) for each area/concern of potential noncompliance reported by complainant into the CMS database.

  • Provide technical assistance to assist facility administration and/or staff with rule interpretation, application, or issues of concern.

  • Prepare testimony including affidavits for hearings and court proceedings. May be called upon by the court to provide evidence and serve as expert witnesses in support of decisions made.

Working conditions

  • We are hiring for multiple locations, please indicate in the questionnaire during your application process which location you prefer.

  • Hybrid remote/on-site work is required for this position. The first few months will need to be in the Tigard or Salem office to complete required training. Reporting to the Salem, Dallas or Tigard office is required when business or operational needs are dependent on in-person meetings, assignments, or activities.

  • You will primarily work within a variety of statewide long-term care facilities and working conditions (such as small conference rooms, standing for long periods of time, working on a laptop computer, etc.).

  • Lift and carry up to 20 pounds throughout each day.

  • Irregular work schedules and frequent schedule changes, including occasionally working long days, nights, overnight travel, and weekends/holidays.

  • You may interact with a wide range of people including some who have experienced trauma and exhibit challenges managing their emotions.

  • You will be a mandatory reporter of abuse and neglect of children, seniors, and other adults with disabilities.

  • Extensive daily use of electronic devices (computer, wireless communication devices, etc.).

  • You will be required to frequently travel and drive statewide, at times, in inclement weather, long distances, and overnight for variable lengths of time to and from NFs, ODHS offices, for meetings, training, and surveys.

  • State and rental cars are available for use as well as mileage, accommodation and meal reimbursement are provided.

  • This position is not eligible for overtime. However, straight time is awarded for use in addition to other paid time off.
