Job Description:

Nursing Care Partners are unlicensed staff members who are accountable to, and work under the direct supervision of a professional nurse to implement delegated aspects of nursing care. Provide patient care, support the admissions and discharge process, and comply with patient safety practices under the direction of the professional nurse. Observe patients' physical, mental, and emotional conditions and report any change to the nursing.

SUMMARY Responsible for coordinating patient care for enteral nutrition therapy in an efficient and cost-effective manner. Screens for nutritional problems and notifies Nutrition Support Dietitian of risks. Coordinates deliveries through suppliers, branches and reimbursement teams. ESSENTIAL FUNCTIONS To perform this job successfully, an individual must be able to perform each essential function satisfactorily, with or without reasonable accommodation. * Ensures patient confidentiality at all times. * Communicates as necessary with patients receiving enteral nutrition to identify general health status, check enteral therapy compliance, formula and supply inventory, insurance coverage, provide information about products and services, and provide overall coordination of care and service (custom assessment in CPR+). * Ensures patient information is updated and accurately documented in CPR. Checks for proper HME orders, detailed written orders and authorizations prior to scheduling shipment and communicates with appropriate departments when needed. * Enters new orders into the patient's chart in CPR+ after their review, if warranted, in accordance with Company policy and procedure. * Generates delivery and equipment pick-up tickets * Follows monitoring systems and processes for the operating unit. Provides quality care is in accordance with physician orders and the organization's policies and procedures. * Generates clinical reports from CPR+ as directed. * Documents all patient interactions in CPR+ as progress notes and / or assessments, fulfilling payor requirements for refills. * Coordinates with warehouse and enteral nutrition team to ensure all formulas, equipment and supplies needed are available for patient care. * Refers patient problems or issues to designated departments or assigned dietitian. Communicates and documents details of inquiries or complaints that are outside of job boundaries. * Troubleshoots enteral tube feeding pump problems and incorrect or delayed shipments as needed, and coordinates delivery of new pump or additional formula/supplies with branch if necessary. * Offers ideas for delivering high quality, cost-effective services. * Serves as a resource for other branches about enteral nutrition supplies and servicing processes. * Mentors new employees and assists Team Lead Nutrition Care Coordinator in training. * Participates in nutrition care services directly including patient satisfaction data collection, outcomes measures and performance improvement. * Acknowledges compliance to the organization formulary. * May be a resource for enteral nutrition delivery ticket confirmations, invoices and patient home deliveries to ensure accurate claims. * Communicates with branches when enteral nutrition patients are discharged and local equipment retrieval is required. * Completes all necessary paperwork, forms, and records according to policy. * Maintains solid knowledge of, and complies with, Company policies and procedures. * Performs other duties as assigned. EDUCATION, TRAINING AND EXPERIENCE REQUIREMENTS * High School Diploma or equivalent required. * Minimum of three (3) years of experience in DME/home infusion or in medical facility as a diet technician, LPN, or medical assistant. * Experiential requirements may be waived in lieu of evidence of progressive growth in and attainment of the skills necessary to perform the required duties. KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED The requirements listed below are representative of the knowledge, skill, and/or ability required, with or without reasonable accommodation. * Ability to organize, prioritize, and schedule work assignments. * Ability to foster a cooperative work environment. * Strong interpersonal and communication skills and the ability to work effectively with patients, branch departments, other branches and outside entities. * Knowledge of computerized information systems. * Working proficiency with the Microsoft Office Suite, peripheral devices (scanners, copiers, fax machines) and keyboarding * Knowledge of state, federal and commercial enteral nutrition billing and reimbursement requirements. * Ability to maintain recordkeeping systems and procedures. * Ability to work flexible work schedule. * Ability to manage resources to improve patient care * Ability to work without constant supervision, cognizant of the Clinical Nutrition Manager's supervisory role. Language Skills Ability to read, analyze and interpret clinical information appropriate to duties and responsibilities assigned. Ability to operate information systems, follow policy, complete required forms, and prepare reports. Ability to effectively present information and respond to questions from referral sources, managers, patients, employees, payors and the general public. Mathematical Skills Ability to calculate figures and amounts such as discounts, percentages, volume and weight calculations and volume and weight conversions.
