SSM Health Rehabilitation Hospital is looking for Full-Time-Day (7a - 7p) -Nursing Assistants (CNA - Certified Nursing Assistant) to join our team at our inpatient hospital in Bridgeton, Missouri!

Under the direction of a Registered Nurse, assists patients in the rehabilitation process through provision of appropriate nursing care as well as carryover of rehabilitation techniques.


  • BLS preferred. High school diploma or equivalent preferred. Hospital experience, certification as a nursing assistant preferred

As an Administrative Agent of the Missouri Department of Mental Health (DMH) serves as a major point of entry for people eligible for mental health services funded by DMH and is responsible for serving as gatekeeper to the public mental health system. BJC Behavioral Health also administers the BJC Employee Assistance Program ad Behavioral Health Partners, an administrative services organization. Overview Role Purpose Serves as a liaison between BJC Behavioral Health staff and inpatient staff during hospitalization for Healthcare Home clients across multiple hospital systems. This position will facilitate a smooth transition between hospitalization and returning home by coordinating care and alerting the care team to the follow up needs of the client. This position provides care coordination services designed to streamline plans of care, reduce hospital admissions, ease the transition to long term services and supports, and interrupt patterns of frequent hospital emergency department use. Responsibilities Utilizes electronic databases and tracking systems to identify and alert continuous treatment teams and their assigned Nurse Care Manager to Healthcare Home client hospital admissions.Reviews and investigates hospital records across multiple systems during hospitalization. Collaborates with physicians, nurses, social workers and others to continue implementation of the treatment plan. Informs care team of important information during hospitalization including but not limited to: referrals made, inpatient status, follow up needs, high risk situations, and make recommendations for aftercare.Participates in after care planning during hospitalizations for clients deemed high risk or high utilizers of hospital/ER services.Collaborates with hospital staff regarding aftercare plans and needs. Focuses on increasing clients' Ability to manage care and live safely in the community in order to shift from use of reactive care and treatment to proactive health promotion and self-management.BJC has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.
