The Care Partner is a support care provider who functions under the direction of the Registered Nurse. The Care Partner primary focus will be to promote a positive patient experience by partnering with the nurse, physician, healthcare team and other healthcare providers to provide excellence in patient experience; and assist with daily living activities and comfort measures.

The Care Partner assists other health care providers with safe, age appropriate and culturally aware care by performing basic patient care functions; i.e. ADL's, ambulation, and vital signs.

The Care Partner may require approved advanced technical training skills may be required i.e. phlebotomy, simple treatments, dressing changes, oral suctioning, and discontinuing Foley catheters. Performing additional advanced skills may be required to support clinical care and will fluctuate depending on the needs of the unit.

Performing clerical functions such as: retrieval of data, maintaining unit supplies, and transporting supplies/patient medications will be required. The Care Partner must be able to work in a fast-paced environment which requires multi-tasking and effective communication with the Registered Nurse and ancillary team. Must be able to perform under stressful conditions with a compassionate, respectful and service mindset.

Must be able to take direction from a variety of care providers. Must be organized with excellent communication skills.


Required Minimum Education

  • Rising High School Senior (completion of junior year) or senior classification in with satisfactory standing (2.0 GPA or higher)

Preferred Education:

  • High school diploma or General Education Diploma/Development (GED). Graduation from a State approved program for Certified Nursing Assistants or Patient Care Tech Program preferred.

Required Minimum Certification

  • Basic Life Support (BLS) certification through American Heart Association (AHA) is required by the first day of employment.

Required Skill

  • Ability to follow detailed directions.

  • Strong interpersonal skills.

  • Clear verbal and written communication, specifically ability to document.

  • Ability to work individually and as part of a team.

VIEW JOBS 12/29/2022 12:00:00 AM 2023-03-29T00:00 <p>Purpose</p><p>To perform non-professional direct patient care duties under the supervision of nursing personnel and to assist in maintaining a positive physical, social, emotional and spiritual environment for the residents.</p><p>Essential Functions</p><p>Job Functions</p><ul><li><p>Assist residents with medications as ordered by their physician(s).</p></li><li><p>Documents all functions related to medication management as required by regulation, and by community protocols as defined in the medication training program.</p></li><li><p>Obtains, receives, and stores medication(s) from pharmacy.</p></li><li><p>Participates in the medication training program and demonstrates competency per the competency checklist.</p></li><li><p>Maintains order, cleanliness in medication storage area.</p></li><li><p>Assist residents with daily bath, dressing, grooming, dental care, bowel and bladder functions, preparation for activities within the community, reminders of medical appointments, and basic room care.</p></li><li><p>Provide functions as directed by supervisor including daily and ongoing records including vital signs, weight and measure residents, and observe and report presence of skin breakdowns; review care plans daily; report changes in resident conditions to supervisor ; and, record all necessary charting entries and report all accidents and incidents.</p></li><li><p>Prepare residents for meals and snacks and perform after-meal resident care.</p></li><li><p>Maintain resident confidentiality; treat residents with kindness, dignity and respect; know and comply with Residents' Rights rules; and promptly report all resident complaints, accidents and incidents to supervisor.</p></li><li><p>Assist as directed in proper admission, transfer and discharge of residents, inventory resident possessions and report defective equipment to administration.</p></li><li><p>Understand and use Universal Precautions and follow established infections control, hazardous communication and other safety rules; ensure cleanliness of assigned residents' rooms; properly maintain and record resident restraints; and, promptly report all violations of safety and sanitation rules to supervisor.</p></li><li><p>Attend and participate in orientation, training, educational activities and staff meeting.</p></li><li><p>Any other tasks, assignments, projects or requests as deemed by management.</p></li></ul><p>Qualifications</p><ul><li><p>High School education preferred.</p></li><li><p>Must be 18 years of age or older.</p></li><li><p>Must read, write, speak and understand English.</p></li><li><p>Completion of training program for certification.</p></li><li><p>Previous experience with geriatric nursing preferred.</p></li><li><p>Knowledge of procedures and techniques involved in administering basic first aid and resident care.</p></li><li><p>Basic understanding of aseptic and sterile techniques.</p></li><li><p>Good physical and mental health; neat, clean, well groomed and responsible.</p></li><li><p>Patience, tact, enthusiasm and positive attitude toward the elderly.</p></li><li><p>Must have upper body strength adequate to bend, lift, shift, move, and/or assist in moving articles of more than twenty-five pounds.</p></li><li><p>Must be able to squat, reach, stretch without distress, Must be able to tolerate extended periods walking, standing.</p></li></ul> Manor Lake Hiram Hiram GA
