his position is in our Family Medicine department in our Salmon Creek location.

The experience at The Vancouver Clinic begins each time our patients or their families have an encounter with a TVC team member and continues throughout their visit. Whether you provide direct or indirect patient care, we want our patients to feel that they are in a welcoming place where they receive the best customer service and patient care available.

In this position, you will:

  • Provide professional, compassionate evidence-based nursing care for TVC patients.

  • Provide care based on the nursing process; assist patients in treating illness and maintaining health.

  • Apply assessment skills and provide professional nursing care delivery that promotes patients' optimal health and independence.

  • Educate patients and families in healthcare issues.

  • Act as patient advocate.

  • Facilitate communications between provider and community-based healthcare programs.

  • Be required to have professional nursing experience that includes a broad range of general nursing knowledge and expertise.

  • Develop and validate knowledge and skills essential to the assigned department.

  • Perform procedures pertinent to department assigned.

  • Participate in clinical teams and may delegate to other health care workers.

Required qualifications for this position include:

  • Current, unencumbered Washington RN license.

  • American Heart Association Basic Life Support (AHA BLS) for Health Care Provider certification at time of hire. ACLS or PALS may be required.

  • Relevant training and certifications applicable to the position.

  • Act as a mentor, role model, and resource for co-workers.

  • Be dedicated to meeting the expectations of patients and co-workers.

  • Interact effectively with patients and physicians in pressure situations while maintaining tact and diplomacy.

  • Work independently while functioning as a team member.

  • Organize and prioritize workflow.

Preferred qualifications for this position include:

  • Bachelor's Degree in Nursing preferred.

  • Excellent written and verbal communication skills.

  • Experience in an outpatient medical setting.

  • Experience with an electronic health record, preferably Epic.

We are the largest multi-specialty group in Southwest Washington, and growing. The ideal candidate will be caring, positive, flexible, and team-oriented, and work with kindness and professionalism at all times . ESSENTIAL FUNCTIONS AND RESPONSIBILITIES Patient Care * Coordinate care for high acuity patients (case management), based on provider's orders, community resources and patient preference. * Triage and prioritize patient needs based on nursing assessment, availability of resources and patient preference. * Obtain all pertinent new patient information and diagnostics prior to consult if applicable (H&P, Medications, Labs, films) * Possess working knowledge of community resources to assist patients in meeting health care needs. * Perform, assist, and delegate procedures and injections as ordered by provider. * Perform procedures pertinent to specialty where assigned. (May include treadmills, PFT, holters, IV therapy, surgical assisting, EKGs, injections, and others). * Perform nurse visits. * Provide and direct patient education, advice, and procedures as appropriate. * Assist patient in obtaining necessary appointments and testing to meet his/her needs. * Facilitates additional labs/tests according to provider orders/protocols. Communication * Work the EMR in-basket for test results, prescription refills, MyChart communications, prior authorizations and telephone encounters. * Process Epic MyChart communications in a timely manner that aligns with organizational expectations. * Initiate alternate communication method with provider regarding high priority patient care to ensure same day completion. * Triage voicemail messages to meet needs of acutely ill patients first. * Answer voice mail in same day received, unless message is left after 4:30 PM. * Return all voice mail left after 4:30 PM within the first two hours of the next working day. * Facilitate communications between provider and community-based health care programs (nursing homes, Home Health, Hospice, hospital, and others). * Instruct Medical Assistants in procedures and care as necessary. Leadership * Acts as a mentor, role model and resource for co-workers. * Delegate responsibility to facilitate patient care. EDUCATION AND SKILLS * A graduate of accredited school of nursing. * Current unencumbered Washington State RN license. * Excellent written and oral communication skills. * Ability to interact effectively with patients and providers in high-pressure situations while maintaining tact and diplomacy. * Ability to work independently while functioning as a team member. * Ability to organize and prioritize workflow independently. * Ability to direct others in urgent and emergency situations. * Experience with an electronic health record, Epic experience strongly preferred.
