The RN-PCN nurse interprets the philosophy and goals of the CLC and Nursing Service to others in the work setting; functions within established guidelines and demonstrates the ability to work effectively and develop sound relationships with residents, their caregivers and other professionals.
Work Schedule: Mondays thru Fridays, from 7:30 am to 4:00 pm. May include holidays, weekends and other tours of duty as needed to cover patient care needs.

The RN-PCN works in close coordination and collaboration with the Assistant Nurse Manager and Nurse Manager.
The Permanent Charge Nurse (PCN) is a registered professional nurse who is accountable to the Nurse Manager. He/she functions as a day to day clinical leader, directing and guiding the nursing staff to assure, safe high-quality care and rendered.
This includes the management, supervision and evaluation of care delivered by nursing and other allied health care staff. As a registered nurse, the Permanent Charge Nurse is also responsible for contributing to the establishment and implementation of evidence based nursing standards of practice. Creates a positive work environment which enhances care delivery and professional development of clinically competent staff.
Promotes, creates, demonstrates and ensures a culturally diverse workforce in which the values and needs of all individuals are respected. As RN-PCN he/she is responsible for planning holistic comprehensive person care focusing on an interdisciplinary approach. He/ She maintains expertise in skilled nursing care, rehabilitation, restorative care, long term care spinal care injury and diseases and hospice and palliative care.
He/ She is closely involved in planning and design of the environment of care.
He/she supports an environment where the presence of plants, pets, and children are integrated bringing vitality to both public and private areas of a home like CLC. He/she supports an environment that is de-institutionalized. He/she possess knowledge of the Joint Commission Standards for Nursing Care Centers (NCC), the application to the comprehensive Minimum Date Set (MOS) assessments process and restorative care modalities.
The RN-PCN initiates improvement in care delivery and identifies and resolves problems impacting client care and unit operations. Key responsibilities include:
Empowering staff to recognize their role in the system;
Creating an atmosphere where caring for clients is the foremost concern;
Coordinating and as necessary performing the provision of safe and high quality care assisting staff to fulfill performance expectations;
Collaborating with the appropriate manager/administrator to develop, implement and evaluate goals and objects for the area (s), and
Ensuring care plans reflect the individualized needs of each resident and that they are utilized as the road map to resident's care. Revises care plans according to changes in condition and medical orders.

Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Must be proficient in spoken and written English. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office.
This position involves lifting, moving, and transporting of patients.
The position involves exposure to blood borne pathogens, fine manipulation, standing, walking, bending, crouching, and reaching patients.This position involves traveling distances greater than 20 miles and driving a vehicle. It requires dealing with the acutely ill, with patients who may display disturbed behaviors, and with stressful situations. Assisting with transferring patients to wheelchairs and/or may need to escort a patient to other areas.
Moving patients and weighing patients in excess of 50 kg. or more. Wearing of a respirator may be required at times in the performance of the duties of this position. When required to use respirator, employee cannot have facial hair or any condition that interferes with the face to face piece seal or valve function of the respirator.
