Assumes responsibility for providing skilled, creative, high quality nursing care to patients under the direction and supervision of a registered nurse. Performs a variety of direct and indirect patient care duties and responsibilities in collaboration with other nursing staff. Is accountable for safely managing the administration of medication and monitoring effectiveness.

Responsibilities

Essential Functions/Key Responsibilities:

1.Records and reports all observed symptoms, reactions, treatments, and changes in the patient?s safety status to the registered nurse.
2.Collaborates with other members of the team in providing health information and maintenance of the medical record.
3.Monitors the patient milieu for safety issues and assists with promoting a positive therapeutic environment.
4.Responds to patient care needs and issues, assists patients with personal hygiene and other activities of daily living. Collaborates with and directs other nursing staff to assist with any patient nursing needs.
5.Assists in the admitting, discharging and transferring process ensuring that patient safety is addressed.
6.Is expected to participate in treatment team meetings, assist with development of care plans, and reports directly to the registered nurse any issues for shift report.
7.Administers medications to assigned patients (LPN?s approved for administration of medications must have satisfactorily completed a medication course for LPNs and approved by the In-service Department for administration of medications).
8.Complies with agency personnel policies and procedures as, well as with policies and procedures specific to the facility.
9.Other duties as assigned.

Qualifications

Valid and current Massachusetts licensure as a Licensed Practical Nurse, or registered nurse, current certification in CPR. Experience with addiction treatment preferred.

Must demonstrate ability to relate to a substance abuse treatment population in a sensitive manner, an interest and willingness to work in a multicultural setting and must share a customer service orientation. Identifies and resolves problems in a timely manner; Uses reason even when dealing with emotional topics. Manages difficult or emotional client situations; Responds promptly to client needs and maintains confidentiality.

Provides nursing care for patients by following professional, hospital, departmental and certification standards in order to maintain quality care. Identifies and addresses patient needs by performing nursing assessments, developing nursing interventions and participating in interdisciplinary team meetings to facilitate discharge. Performs duties of medication and treatment nurse following pertinent policies and procedures in order to carry out physician's orders in a safe and therapeutic manner. Works with patients by providing instruction to facilitate the independent management of their personal, hygiene and health needs. Supervises employees of a lesser grade by assigning tasks, based on individual competencies, evaluating performance and making recommendations concerning performance to maintain a high level of quality care. Provides instruction to other nursing staff by example and explanation to ensure quality services to patients and to maintain professional standards of nursing. Facilitates the implementation of treatment plans by interdisciplinary team by conferring with appropriate health care professionals in order to meet the stated goal for each patient. Admits, transfers and discharges patients, coordinating health care services to ensure continuity of care following standards established by hospital policy and certification guidelines. Identifies, evaluates and responds to changes in patient conditions, documenting and reporting such changes to the appropriate person to ensure prompt response to patient needs. Maintains an awareness of the D.M.H. regulations and hospital policy and procedure regarding the use of seclusion and restraint, through supervision and instruction, to maintain patient dignity and human rights. Maintains professional proficiencies by attending and participating in inservice trainings and educational programs to foster growth and development. Assumes charge nurse responsibility for a single shift in the absence of a nurse of a higher grade to provide continuity of care. Provides psychosocial support to patients through individual counseling and group work to maintain the therapeutic milieu. Performs tasks of assigned staff in their absence to ensure continuity of care. Provides for the supervision of staff through individual counseling and/or regular staff meetings; through appropriate role modeling and overseeing the orientation process. Participates in the progressive disciplinary process as required. Actively involved in the improved operational process at both unit and hospital wide level to improve quality of care.
