The Home for Little Wanderers is opening Unity House, a new community based group home in Keene, NH, serving youth ages 14-18, who identify as members of the LGBTQ+ community. The program is the first of its kind in the Granite State, and offers a safe, supportive living environment with 24-hour staffing for up to eight youth, while they prepare for family reunification, independent living, secondary education paths and future self-sufficiency.

Position Description

Under the direction of the Director of Nursing and Program Director, the Registered Nurse will provide nursing care and manage the ongoing health care of residential and/or day students in assigned program(s). The Registered Nurse will also provide additional duties identified in the Caring Together contracts and will function in the role of "Nurse Monitor" as part of the Medication Assistance Program (MAP)

Essential Functions

  • Develop an individualized health services plan using a collaborative, wellness focused approach. Monitor the implementation and effectiveness of the plan.

  • In collaboration with the Administrative Team, develop and ensure implementation of a comprehensive health/wellness component to program planning, including environmental, nutritional, exercise and fitness

  • Provide ongoing Quality Assurance by monitoring a MAP site's medication administration system to ensure compliance with DPH, DMH and DCF regulations, licensing requirements and MAP policies .This is accomplished by conducting reviews of medication administration practices at regular intervals

  • Oversee all aspects of the administration of medications, including the direct administration, training and supervision of licensed and unlicensed staff designated to administer medications, and the preparation of leave of absence medications. Assure compliance with all regulatory guidelines. May be asked to serve as a MAP Certified Trainer.

  • Observes, no less than once every 12 months, all Certified staff employed at the site prepare and administer medication in a manner consistent with all MAP policies, procedures and protocols. Such observation can be either a classroom-type (mock) demonstration or an actual administration on site. The observation shall be documented on a form developed by the agency that contains the following information: Certified staff name, Certification number, date of observation, Nurse Monitor signature.

  • Oversee the nursing assessment and ongoing evaluation of clients including vital signs and anthropometric measurements, as prescribed. Assist in making decisions regarding the course of treatment (i.e. whether to exclude a child from classes or take a child to the emergency room), and monitoring the effectiveness, side effects, adverse reactions, and drug interactions of all medications.

  • Monitor laboratory reports and initiate appropriate action

  • Oversee the coordination of medical, diagnostic and laboratory appointments. Conduct annual health screening procedures and assure that all immunizations are current. Assure compliance with all regulatory guidelines.

  • Coordinate the delivery of health care with ancillary staff, family members, guardians, psychiatrists, primary and specialty health care providers, and other professionals within the Home and with external organizations. Maintain on-going communications with primary health care provider.

  • May be required to administer medication outside of role of Nurse Monitor, including insulin and/or other injectable medications, under nursing license.

  • Work actively as part of an interdisciplinary team to provide quality care, attend team meetings and conferences, and communicate regularly with team members

  • Complete reports and documentation needed for client records, the Home and collateral and regulatory agencies

  • Provide and coordinate health education for clients and families and training for staff

  • Assure that all medication and medical supplies are ordered, received and checked for accuracy, monitor supply budget

  • May be required to carry a mobile device

  • Perform other duties as assigned

Education and Experience

  • Equivalent of three years pediatric and/or psychiatric nursing experience and Bachelor of Science Degree in Nursing

  • Certified in CPR and Standard First Aid; MAP Trainer

  • Computer literate including Microsoft Office and Internet Explorer with the ability to learn new software applications

Physical Requirements

Ability to communicate effectively verbally and in writing. Ability to follow instructions. Ability to restrain clients in an emergency situation.
