This Nurse Manager is responsible for overseeing the medical needs of BrainCare residential participants including medication management and providing hands-on skilled care duties. Working closely with the Director and other members of the care plan team, the Nurse Manager will make critical recommendations for care planning and training staff on how to provide proper care to all participants in the program. Acts as liaison between the care planning team, the primary care physician and coordinates other needs such as medication changes, durable medical equipment and other long-term Medicaid benefits.
ESSENTIAL JOB FUNCTIONS
Oversight of all medical requirements for SLP certification, per state regulations, including but not limited to, medication administration, medical care coordination, medical assessment and hands-on care with activities of daily living (ADLs).
Attends care planning meetings to provide input on medical needs to the care planning team and to report back on information given at weekly meetings with the Primary Care Physician (Rocky Mountain Senior Care).
Trains staff on proper care techniques to maintain/improve quality of services.
Available to answer questions from residential staff regarding medical concerns as well as provides guidance for situations in which a participant may need to be hospitalized.
Involved in intake assessments for potential participants and leads/participates in medical assessments as needed.
Performs and supervises the administration of prescribed medications and treatments in accordance with nursing standards, licensure, state and federal regulations.
Signs off on and implements medical orders, orders medications from pharmacy, and disseminates orders and instructions from the Primary Care Physician to appropriate personnel.
Oversight of the appointment summaries, health tracking and medical record portions of the Electronic Health Record software, Therap, as well as audits medication administration charts monthly.
Instructs participants and family regarding medications and treatment instructions/options.
Follows up on lab results as needed. Maintains communication regarding results with all facilities (e.g. physician's office, hospital intake and discharge, pharmacy).
Sets up and coaches participants with self-administration medications as per policy and procedure.
Ensures that medications for participants are verified to be correct upon arrival from pharmacy.
Participates in the rotation of manager on call (MOC).

Organizes and prioritizes while working with strict deadlines
Responsible for the overall supervision of LPNs, BrainCare Navigators, C.N.A.s and QMAP Leads, including orientation, counseling, training, scheduling, disciplinary actions (with HR assistance).
Provides guidance and regular coaching to employees
Assigns tasks fairly and appropriate to job responsibilities
Required to maintain detailed supervisory notes documenting counseling and coaching interactions.
Must address disciplinary issues in a timely manner applying Company policy
Ensures that performance evaluations are conducted thoroughly and in a timely manner
Communicates goals and expectations clearly and effectively with direct reports
QUALIFICATIONS AND REQUIREMENTS
Must be a registered nurse with a current Colorado Registered Nurse License
Three (3) to five (5) years of healthcare experience required, with general knowledge of medical care, acute care, and experience working with challenging behaviors preferred
Minimum one (1) year experience of supervisory or team management experience
Excellent time management skills, a positive attitude and ability to lead a team
Experience with working in the Medicaid and Medicare systems is highly desired
Current BLS certification through the American Heart Association is required
Unrestricted driver's license and motor vehicle insurance required
