The Duke Department of Obstetrics and Gynecology is seeking an Advanced Practice Provider to join our team of experienced Complex Menopause providers who provide informed care and counseling for midlife women tailored to their specific needs and concerns. The team currently includes generalist OBGYNs with Menopause Society certification who will provide mentorship and oversight. The clinic location are in Durham, NC and Morrisville, NC.

Primary hours are Monday through Friday with hours flexible to meet the needs of the patients. Telehealth sessions are part of the weekly schedule in clinic or remote based on the day's schedule. One Saturday half day session per month is required. Responsibilities limited to the ambulatory environment without in-patient or call duties.

Menopause Patient Care

The APP will be responsible for management of gynecological patients for care and/or treatment of common menopausal problems including but not limited to

  • Menopause and Menopausal Hormone Therapy: Natural Menopause, Premature Menopause, Early Menopause, Surgical or Radiation-Induced Menopause

  • Symptomatic Perimenopause: hormonal contraception, IUD, Nexplanon, cyclic progestin

  • Management of menopausal/perimenopausal symptoms in hereditary breast and ovarian cancer patients and those at high risk

  • Symptomatic High-Risk Cancer Patients and Survivors: Hormonal and Non-Hormonal Management

  • Use of bioidentical hormone therapy (BHT) if desired

  • Management of menopausal/perimenopausal symptoms with high risk disorders; Smokers, history clotting disorders, existing cardiovascular disease (stroke, CAD), valvular disease, liver disorders, transplant patients

  • Assessment of abnormal and postmenopausal bleeding

  • Mental health disorders in menopause and perimenopause: Menopause-Related Cognitive Changes and Mood Changes

  • Management of Genitourinary Syndrome of Menopause

  • Non-surgical management of Prolapse/Urinary Incontinence (pessary)

  • Midlife Vulvar Dermatoses and Vulvovaginitis: Treatment of lichen sclerosus, DIV, lichen simplex chronicus, GVHD, and lichen planus; recurrent bacterial vaginosis and yeast vaginitis

  • Use of Estrogen Receptor Agonists/Antagonists/ "Designer Estrogens"

  • Midlife Weight Management with anti-obesity medications (AOM) and dietary and lifestyle modifications (plant-based, Mediterranean diet)

  • Sexual health/sexual dysfunction: Genitourinary syndrome of menopause; Sexual desire disorders with Flibanserin (Addyi), Sildenafil/Cialis, topical testosterone, vaginal DHEA; Sexual arousal disorders/orgasm disorders; Pelvic floor dysfunction

Gynecological Care

The APP will be responsible for management of gynecological patients for routine care and/or treatment of common gynecological problems including but not limited to:

  • All methods of family planning and counseling for reproductive life planning including IUD Placement and removal and Nexplanon insertion and removal

  • Routine screening exams and pap smears

  • Referrals for appropriate screening tests (mammography, Dexa, colon screening)

  • Screening and treatment for sexually transmitted infections

  • Breast changes or masses

  • Pelvic/abdominal pain

  • Routine Postoperative follow-up and problem visits

  • Cervical and vulvar biopsies

  • Treatment of vulvar cysts and abscesses

  • Endometrial biopsies

Miscellaneous

  • Analyze clinical practice patterns so as to provide the best medical care and to increase effectiveness and efficiency.

  • Communicate verbally and in writing all pertinent patient information to supervising physician, nursing staff as well as other relevant healthcare providers.

  • Collaborate with primary care providers, endocrinology, cardiology, oncology, mental health, and sexual health providers to provide care tailored to the patient's needs and risk factors.

  • Provide timely documentation in the electronic medical record of all relevant data including results of diagnostic tests; laboratory results; patients' condition and response to therapies/interventions and communications with physician/advanced practice provider colleagues.

  • Provide timely response to patients Maestro Care and phone messages.

  • Precept learners from a variety of disciplines.

Job Eligibility Requirements

Education

  • Nurse Practitioners: Requires completion of an accredited Nurse Practitioner program, with a Master's degree in Nursing.

  • Physician Assistants: Completion of an accredited Physician Assistant program. A Bachelor's degree required; Master's degree strongly preferred.

Certification

  • BLS required

License

  • Nurse Practitioners: Must have current or compact RN licensure in the state of North Carolina. BLS required. National certification as a NP required prior to or within 6 months of employment.

  • Physician Assistants: Must be licensed in the state of NC, and be certified as a Physician Assistant by the National Commission on Certification of Physician Assistants.

Experience

  • Prefer 1+ years of work experience as a Nurse Practitioner or Physician Assistant in Women's Health

  • Prefer candidates with experience in management of midlife women's health concerns.

  • Experience/training in sexual health management and/or prescribing anti-obesity pharmacologics experience is a plus.

  • If not already certified, the expectation would be to obtain a certificate as a Menopause society certified practitioner within the first year.
