Nurse Practitioner (Yonkers, NY, Us)

The Nurse Practitioner (NP) will execute tasks set forth by the CMO/CKCC entity on patients attributed to that entity and to the aligned nephrologists practicing in the Montefiore/Einstein Renal Division and Kidney Medical Associates (KMA).

Responsibilities, seeing patients to : 1) complete Comprehensive Health Evaluations (CHE), PHQ9 depressions screening, and Patient activation measures, 2)provide modality training, 3) conduct post-acute transitions of care visits, and 4).

The NP will work with CMO care teams and the aligned nephrologists to increase optimal starts, transplant nephrology referrals, arm access evaluations. The NP will also help with TOC visits (after hospital discharges) through tele-visits or in person and will also participants on high risk interdisciplinary rounds on patients with the CMO and aligned nephrologists.

The NP will participate in training on how to administer and bill for necessary enrollment consents and documents, completing patient activation measure and comprehensive health evaluations and devising programs that best meet the benchmarks put forth by Centers for Medicare and Medicaid Services. Travel to dialysis facilities, various ambulatory sites and hospitals will be required for this position.

Requirements:

Bachelor's Degree required, Master's Degree preferred

License to practice as a Nurse Practitioner in New York State is required.

Minimum of 5 years experience as a Nurse Practitioner

• Experience in clinical research studies involving human subjects

• Familiarity with database utilization, regulatory compliance and human subjects protection issues.

• Knowledge of standards of good clinical practice.

• Knowledge of chronic kidney disease; BP control and essentials of diabetic control.

• Some Knowledge of care for patients receiving peritoneal or home dialysis
