Are you a Nurse Practitioner that is highly motivated and technologically savvy?

Christiana Care Health System is recruiting for a Nurse Practitioner to join our Christiana Care Virtual Primary Care Practice at our CareLink Building, located near the Wilmington Hospital, in the City of Wilmington. We are looking for a Nurse Practitioner that can work a flexible schedule between the hours of 9am-9pm with periodic weekend coverage needed. This innovative primary care model will allow for high adult care delivery through virtual web-based and telephonic visits. This position will have the support onsite with a collaborating physician, nurse, and medical assistant. Additionally, they will have the extensive collaborating support of the CareLink team members.


  • Provides direct management and ambulatory care of patients under physician supervision Identifies each patient prior providing care and treatment

  • Assesses and documents patient response to intervention

  • Demonstrates ability to carry out critical skill as outlined in policies and procedures

  • Medication reconciliation and prescriptions done safely

  • Ability to independently review patient problems and create a plan of care

  • Ability to refer patients for appropriate testing, review and report results as necessary

  • Providing patient education and counseling to patients

  • Participate in chronic disease management of patients

  • Performs assigned work safely, adhering to established departmental safety rules and practices

  • Reports to the appropriate personnel, in a timely fashion, any unsafe activities, hazards, or safety violations that may cause injury to oneself, other employees, patients, and visitors

  • Demonstrates skills and knowledge necessary to provide care appropriate to adult patients


Licensure or license eligible for Registered Nurse and Advanced Practice Nursing as set forth by the Delaware Board of Nursing, as well as Maryland, Pennsylvania, and New Jersey, which you will be reimbursed for.

Certification by the appropriate national organization specific to area of specialty

Prescriptive authority

Certified in BLS

The Pediatric Nurse Practitioner (PNP) is responsible for providing and coordinating direct and indirect primary pediatric care to patients and their families in the outpatient, ambulatory care setting, using a case management approach, and utilizing appropriate services and support. The schedule is Wed 8AM – 4:30PM, Thu 10AM – 6:30PM, Fri 8AM – 4:30PM. Saturday and Sunday 8:00AM to 2:00PM approximately 1 in 7 weeks. Essential Functions: * Provides and coordinates primary health care for children from birth to eighteen years of age. Provides telephone consultation and triage for patients during the normal working hours and during scheduled on-call rotations. * Maintains a current knowledge base through participation in appropriate professional activities, conferences and organizations. Disseminates nursing and medical research findings and incorporates these findings into primary pediatric practice. * Maintains patient records which accurately reflect management of care. * Performs appropriate assessment including history taking, physical examination and developmental assessment of child. * Diagnoses and treats common childhood illnesses, and follows up to evaluate response to treatment. Uses prescriptive authority if appropriately credentialed to do so. * Participates in Quality Improvement programs and works with department management to improve the services provided. * Utilizes appropriate medical staff for purposes of consultation and referral when necessary. * Identifies child/family need for further resources and initiates appropriate referrals. Collaborates with other health care professionals and community agencies in a team approach to comprehensive care. * Provides anticipatory guidance and patient and family teaching based on identification of learning needs. * Acts as a role model to other members of the health care team. Serves as a consultant for colleagues when appropriate both intra- and extra-departmentally, and in the community.
