Responsibilities

About the Position:

Type: Full Time

Location: Home, SNF, Telehealth and other community environments in: Bristol, CT 06010

Hours: Days, Evening, no weekend or on-call committment

Salary: Starting base from $115,000 + year of experience + additional incentive on completed CHAs.

Matrix Provider - What to Expect:

  • Make a difference in people's lives by conducting Pediatric/Adult/Geriatric. assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols.

  • Work collaboratively with physicians, case managers, social workers, family members, key caregivers and any appropriate ancillary medical personnel as appropriate. Collaborate with Primary Care Physician (PCP) on patient education. Provide follow-up.

  • A nationwide network of advanced practice providers to build and maintain relationships.

  • Services may be provided in a variety of venues to include; Home Visits, Skilled Facility Visits, Mobile Clinic Visits, Telehealth Visits, and Retail Clinic Visits.

Qualifications

Must Haves:

  • Master's Degree required OR commensurate experience and satisfactory completion of NP licensure.

  • Current RN and NP licensure in state of practice to include prescription authority or the ability to obtainprescriptive authority.

  • Board certified by the AANP, ANCC or the AACCN in a Matrix approved specialty.

  • Have a current BLS,ACLS or CPR certification.

  • Have a Minimum of 1 year experience as a Nurse Practitioner

  • Have the ability to travel, a valid state driver license and able to drive a car, proof of adequate automobile insurance coverage for the state of residence.

  • Have strong computer skills and familiarity with Employee health/medical record software.

  • Possess excellent verbal and written communication skills with patient, clients and colleagues.

  • Be comfortable and flexible with frequent change.

This is a place where you can stay way ahead as you do your life's best work.(sm) ProHealth Physicians has an opening for a hospital based Utilization Management Review Nurse to join our team. The hospital is located in Bristol, CT Primary Responsibilities: * Daily rounds, reviewing of medical records, concurrent review, and discharge planning on inpatient, and skilled nursing admissions with arrival at facilities in the morning ideally between 0800 and 0900 * Prior authorization on Durable Medical Equipment (DME), Home Health services, and Ambulance transport * Daily inpatient rounds with hospital staff (Hospitalists and Care Managers) * Communicates discharges to the IPA Provider by 12 noon * Reviews inpatient cases with Director daily * Documents Milliman Guideline(s) on every inpatient case within 24 hours of receipt and updates guideline as hospitalization continues * Notifies patient's Primary Care Physician of admission, discharge and discharge plan within 24 hours of each event as appropriate * Analyzes medical cases for appropriateness and plans and coordinates all phases of care in conjunction with the Hospitalist, attending Physician, Specialists both in the inpatient and outpatient settings * Utilizes the nursing process to establish individual patient goals and measures patients' response to the plan of care for appropriate outcomes and intervenes appropriately * Collaborates with physicians, patients, and families to formulate an optimal plan of care that meets all the medical, psychosocial and financial needs of the patient * Explores care options to reduce cost, decrease length of stay, curtail re-admission and influence quality outcomes * Understands capitation vs. fee for service * Knowledgeable regarding network providers and current contracted specialists * Manages cases according to delegation agreement requirements, including notifications regarding LOS >5 days * Regularly communicates with Health Plans as appropriate * Understands health plan benefits and contractual information * Generates authorizations utilizing principles of UM * Accurately uses database for daily documentation of clinical information, opportunity days, denied days and levels of care * Follows policy and procedure for Denials and enters this information into the database with 24 hours of the event * Assists with implementation of all policies and procedures in the group through knowledge and experience * Meets all health plan contract compliance directives for utilization * Maintains ongoing communication and feedback with customers and/or clients in order to support needs and resolve problems. * Attends weekly/monthly group committees as requested * Manages error reports on a weekly basis * Communicates activities and progress of assigned committees and teams to appropriate staff * Identifies and refers members to Case Management prior to discharge or as deemed appropriate through referral review * Participates in IPA Grand Rounds providing a concise history and overview of the member's progress and discharge plan. Employees must comply with any state and local masking orders.
