GENERAL SUMMARY/ OVERVIEW STATEMENT:
The Psychiatry APTM (NP) is an expert practitioner, executing position responsibilities that demonstrate leadership, experience, and creative approaches to the management of complex and investigational patient care within the department of psychiatry. In this role, the NP is a key member of the leadership team and has a responsibility for programmatic oversight and personnel management, as well as a role in the delivery of care to a panel of psychiatry patients across the continuum including; planning, implementation, coordination, and evaluation of psychiatry and co-medical care related outcomes. This position is required to work closely with all members of the team, acting as a liaison between the clinic, urgent care clinic, and the inpatient staff collaborating with members of the multidisciplinary team and primary care providers to deliver longitudinal care to psychiatry patients.

A. Program/Clinical Leadership:
Performs in the role of an Independent Licensed Provider (ILP) and is responsible to work both independently and in collaboration with other providers to maintain a panel of patients. Primary area of responsibility is to develop and maintain an outpatient panel via 7 weekly ambulatory clinic sessions, including Urgent Care as well as being the lead provider supporting patients with serious mental illness enrolled in Medicaid ACO contracts.
Responsible for leading the team in coordinating care and meeting patient care needs in conjunction with a variety of providers including; licensed independent providers acting in, social work and care managers and others both locally and in conjunction with the providers in the communities where the patients live.
Seeks opportunities to disseminate clinical knowledge and nursing practice development.
Leads and/or participates in departmental, hospital and/or system-level committees and task forces as a representative of the department, practice and the Department of Nursing as requested.
Performs other related duties and responsibilities as required.
Assesses, plans, implements, collaborates and evaluates the needs and care of patients.
Performs in the role of Licensed Independent Provider (LIP) and is responsible to work both independently and in collaboration with other providers to maintain a panel of patients.
Promotes self-care, and collaborates with patients to establish realistic health goals.
Provides patient and family teaching, staff education, and consults and collaborates with other health care providers; participates in nursing and medical research.
B. Patient Care Organization
Manifests a high level of expertise in the assessment, diagnosis, and treatment of the complex responses of individuals, families, or communities to actual or potential health problems related to their disease conditions.
Performs physical examinations and provides preventive health measures appropriate to patient needs.
Orders, interprets, and evaluates appropriate lab and diagnostic tests to assess patients' clinical problems and health care needs.
Arranges for appropriate plan of care and follow-up based on outcome of diagnostic, lab, and physical assessment findings. Seeks consultation as appropriate.
Orders medications and writes prescriptions according to organizational and regulatory policies and procedures.
Actively communicates with all member of the patient's care team (i.e. lab values, rejection episodes, med changes, psychosocial issues, etc) quickly, efficiently and thoroughly.
Sets priorities for appropriate and efficient management of patient care that reflects evidence based practice and cost effective management of time, available human resources, supplies and equipment.
Maintains an interdisciplinary approach to planning and communicating patient care information.
Incorporates the concepts of health maintenance, prevention, and promotion through patient education. Assumes responsibility for coordination of patient and family education with, nutrition, social work and pharmacist.
Demonstrates awareness of community resources and assists staff, patients, and families to effectively utilize them.
C. Quality Monitoring
Accountability to develop, implement and oversee a system of performance improvement activities. Uses performance and patient outcome data for continuous quality improvement.
Collaborates with physicians, physician assistants and the interdisciplinary team to refine team goals and objectives and further ensures ongoing continuous improvements to the care delivery model.
Active contributor to clinical innovation as well as leading data informed evidenced based clinical efforts and participant in sharing information in both written publications and oral presentations.
Establishes effective professional relationships with other members of the healthcare team.
Serves as role model and resource for other staff.
Regularly incorporates new scientific findings in specific area of expertise into clinical practice.
Responsible for hiring, orientation and evaluation of Department of Psychiatry nursing staff members, as appropriate.
Develops implements, maintains, and updates staff orientation and training program.
Ensures that annual and other required staff training requirements are met.
Resolves nursing and other personnel issues/problems.
Adheres to hospital and departmental policies. Recommends changes in policies and procedures.
Participates in determining and implementing goals and objectives for the nursing team of the Department of Psychiatry.
Current licensure as a Nurse Practitioner in the Commonwealth of Massachusetts
Certification by a nationally-recognized accrediting body approved by the Massachusetts Board of Registration in Nursing
DEA license and a Massachusetts Controlled Substance Registration number
Current experience with general psychiatry ambulatory patient population as ILP, preferred
Current experience as a ILP independently managing a panel of patients, strongly preferred
3-5 years of Psychiatry experience, preferred
Experience working in a large complex health care organizations.
Experience leading a patient population care delivery model using clinical guidelines and protocols
Ability to work with a variety of information systems and databases: MS Word, Powerpoint, Access

May require occasional work (phone calls, care coordination, patient questions/concerns) outside of normal business hours, including weekends.
