GENERAL SUMMARY/ OVERVIEW STATEMENT:

The Advanced Practice Provider/Nurse Team Lead for the Palliative Care Home Based Program is accountable, in collaboration with the Nursing Director in the Division of Palliative Care and Geriatrics, for the delivery of consistent, high quality patient care, promoting the development and satisfaction of clinical staff, providing operating efficiencies and insuring compliance with hospital and regulatory policies and standards of practice.

This role is an essential leadership position in the hospital organization encompassing patient focused management, optimizing clinical care delivery, engaging team in best practices, and the development of collaborative and team partnerships within units, clinics, community and interdisciplinary settings. The Advanced Practice Provider/Nurse Team Lead will work in collaboration with the Nursing Director, as well as physician, administrative, and other nursing leaders within the division to create a culture of continual education, quality improvement and will seek opportunities for professional development and best practices.

The Advanced Practice Provider/Nurse Team Lead focuses on a targeted area of clinical care delivery within the Division of Palliative Care and Geriatrics/MGH. In doing so, this advanced practice leader will partner with the leadership team on activities in both the inpatient and outpatient settings, along with other key physician, advanced practice, and nursing leaders to facilitate efficient and effective clinical operations.

The Advanced Practice Provider/Nurse Team Lead will join (receive ongoing mentoring from) a team of other APP Team Leads across the ambulatory and inpatient settings. Participation in Leadership Academy courses and participation in local process/quality improvement initiatives will be a key component of this role. The APP team leads will partner with the Nursing Director of the division to establish an APP leadership team, optimal onboarding and mentoring program, and enhanced sharing of best practices and professional development opportunities.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

1.Provides an environment for effective patient-focused advanced practice provider care.

1.1. Responsible for high quality care with positive patient outcomes. Provides independent, direct, and indirect nursing care; performing physical examinations, ordering diagnostic studies, evaluating results and prescribing treatment as appropriate and within the Advance Practice Role as delineated within the state of Massachusetts and MGH.

1.2. Responsible for implementing and evaluating approved policies, procedures and standards of care necessary for the coordination and delivery of excellent patient-focused care.

1.3. Collaborates with the registered nurse, physician, other health care professionals and administrative personnel to plan the delivery of care on the unit/clinic and to create joint protocols for patient care.

1.4. Promotes an environment that is sensitive to the culturally diverse needs of patients and employees.

1.5. Evaluates the outcomes of patient care; uses patient satisfaction information to recommend and integrate appropriate changes.

1.6. Responsible for direct clinical APP practice; preserves the integrity of provider/patient relationship.

1.7. Assists in coordination, development, implementation and evaluation of patient care policies and procedures designed to affect both clinical care and operational efficiency within the division.

2.Personnel Management

2.1. Assists the Nursing Director in the process of recruiting, onboarding, and evaluating staff

2.2. Maintains APP practice for both maintenance of professional licensure and clinical skills.

2.3. Promotes the development of patient care teams engaging unit/clinic/home-based staff in decision -making process.

2.4. Delegates responsibility and authority to appropriate staff.

2.5. Collaborates with nursing and hospital educational resources to facilitate learning experiences for staff and students assigned to the unit.

2.6. Maintains confidentiality, as appropriate, related to personnel management issues.

2.7. Provides ongoing coaching, feedback, and mentoring to 3 APPs at all stages of professional development.

2.8. Contributes to the development of councils and meetings for APP engagement

2.9. Develops education, research, and networking opportunities

3.Administration

3.1. Seeks and acts upon patient, families and staff feedback and initiates appropriate action.

3.2. Promotes, develops and evaluates processes and systems for patient care; advocates for resources and systems improvements when necessary.

3.3. Accountable for compliance with hospital and regulatory standards.

3.4. Negotiates interdepartmental resources, communicates and plans with managers of other patient care units and other department's directors and managers to ensure effective level of service to the unit.

4.Operations Management

4.1. Maintains a safe environment for patient/staff.

4.2. Participates in planning and carrying out to completion projects to improve the physical environment.

4.3. Works collaboratively with the interdisciplinary leadership team to advance efficient and effective operations

5.Collaboration

5.1. Forms partnerships with department heads, physician leaders and other disciplines.

5.2. Works collaboratively with hospital structures for APP leadership

6.Leads and/or participates in performance improvement activities.

6.1. Continually evaluates quality and safety performance, identifies improvement opportunities, and plans and implements programs for improvement.

6.2. Seeks collaboration with community health organizations to achieve positive outcomes and enhance the hospital's community presence.

6.3. Collaborates with Division Quality Director and Nurse Director to promote quality and safety and performance improvement activities. Participates on Quality Committee.

7.Professional Development

7.1. Engages in self-performance appraisal on a regular basis, identifying areas of strength as well as areas for professional/ practice development.

7.2. Identifies learning needs and goals on a regular basis and designs a plan to meet them.

7.3. Maintains necessary clinical through continuing education and direct clinical practice.

7.4. Establishes self as a resource in areas of the hospital where expertise may be required.

7.5. Participates in leadership groups and institutional committees.

7.6. Collaborates with leadership in identification of current and future service needs and shares responsibility for meeting these needs.

7.7. Holds membership in one or more national nursing organizations.

SKILLS/ABILITIES/COMPETENCIES REQUIRED:

Demonstrates ability in domain of Advanced Practice Provider.

Possession of or willingness to develop leadership/managerial skill, which includes interpersonal competence, organizational skills, financial management, personnel management, hardiness and adaptability.

Possession of communication, collaboration, and team-building skills.

Ability to coach and mentor.

Flexibility and involvement in change management experience.

EDUCATION:

Master's or Doctor of Nursing from an accredited nursing program

Licensed as a Certified Nurse Practitioner in Massachusetts

Prescriptive authority for controlled substances in MA (MCSR)

DEA registration for controlled substances

EXPERIENCE:

At least 3 years of Advanced Nursing Practice within the Division of Palliative Care and Geriatrics/MGH, demonstrated ability/desire to grow in a leadership capacity.

SUPERVISORY RESPONSIBILITY (if applicable):

No direct supervision.

Provides support of geographically co-located APP teams (inpatient, outpatient clinics, home-based, geriatrics, community programs) and assists Nursing Director with:

  • Interviewing/hiring of new staff

  • Regularly occurring supervisory meetings with APPs

  • Preparation of/attendance at annual APP performance evaluation

  • Schedule/Payroll

  • APP staff meetings/Division APP Quarterly meetings

FISCAL RESPONSIBILITY (if applicable):

None

WORKING CONDITIONS:

Position may be remote (via telehealth); on inpatient unit; in clinic; in home-based and/or community setting; and may include some travel to offsite practice locations, trainings and meetings.

Individual must have own transportation.

Dedicated program development/leadership time will be provided to support NP Lead responsibilities.
