Job Description Purpose of Position: Under general supervision, serves as a licensed, independent practitioner responsible for planning, coordinating, delivering, and managing comprehensive health care services in a health care setting. Ensures patient and family-centered quality care to improve outcomes and patient experience, in accordance with applicable NYC Health Hospitals policies and procedures, state and federal rules and regulations, and nursing standards of care.

Examples Of Typical Tasks Provides continuous comprehensive medical and nursing care, including preventive and supportive health care services to patients and families. Manages health problems and coordinates health care for various patient populations. Identifies and diagnoses illnesses and physical conditions, and implements therapeutic and corrective measures.

Consistent with the established nursing practice, differentiate between normal and abnormal findings in the screening process to detect changes in health status that require physician’s intervention. Develops and maintains partnerships with community-based organizations and makes referrals to consulting physicians and/or other health care professionals and community agencies, as necessary. Evaluate and interpret patient data, in order to plan and implement appropriate nursing interventions.

Consult with physicians and other health care practitioners in the evaluation of health screening findings to make prospective decisions about treatment that will best support improved outcomes and patient satisfaction. Formulates differential diagnosis and the plan of care based on medical history, physical exams, nursing assessment, and other pertinent information. Identifies health and medical risks and needs.

Develop, implement, and evaluate patient plan of care. Determines effectiveness and modifies as per clinical standards and practice protocols, where needed, to achieve medical and health goals. Orders, conducts and interprets diagnostic and laboratory tests.

Identifies and may prescribe appropriate pharmacological and non-pharmacological interventions and therapies. Manages a patient session schedule allowing for both scheduled and non-emergent unscheduled visits; maintains a work schedule that ensures availability and continuity of care; ensures optimal availability and utilization of appropriate resources. Provide education, guidance, and direction to nursing staff and other members of the health care team to ensure quality and safe medical and nursing care is provided.

Provides education and counseling to patients and families regarding health status, treatment and progress, medication management, disease prevention, and health promotion, records, and maintains a system of relevant documentation. Performs other related duties, as directed. Department Of Surgery-Breast The Nurse Practitioner, under the supervision of the assigned Attending Physician, performs diagnostic and therapeutic tasks and renders varied medical services to provide comprehensive patient care.

Essential Duties Elicits a detailed and accurate history and performs an appropriate physical examination on patients in an outpatient setting. Develop an assessment and a therapeutic plan, and render ongoing patient care based on this plan. Orders and interprets appropriate laboratory tests, X-rays, EKGs, and other diagnostic procedures.

Writes prescriptions according to protocols established by the service and in compliance with state and other governing bodies. Assists the attending surgeon and/or resident surgeons with breast examinations. Performs other specific procedures including, but not necessarily limited to drawing blood specimens for testing, blood counts, urinalysis, pap smears, bacteriologic smears, gram stain culturable material(s), administering oxygen, incisions, and drainage, wound care and debridement, assists with soft tissue biopsies, starting intravenous solutions, administering intravenous medications, and other therapeutic procedures for which the clinician has received specific supervised training and evaluation.

Initiates consultations and refers patients for special testing according to hospital protocols. Participates in the ambulatory breast center practice to observe and record pertinent progress of patients, updates and summarizes charts, writes orders and progress notes, and notifies the Physician of changes in the patient’s condition, when appropriate. Takes appropriate precautions while maintaining patient and staff confidentiality.

Takes appropriate precautions to ensure patient safety. May perform other duties, as assigned. Minimum Qualifications A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and Current certificate as a Nurse Practitioner issued by the NYSED; and Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
