• JOB DESCRIPTION:

Provides acute and comprehensive health care to patients in various states of health or illness. NP practice medicine within their scope of practice based on their education, training and experience. NP collaborate with physicians and other healthcare professionals as needed to provide care to patients.

EDUCATION:

Successful completion of NP educational program. Current AANP Certification required and active licensure to practice as NP by the VT Board of Medical Practice.

EXPERIENCE:

Previous experience as an NP desired. Geriatric experience desired.

This is split position that includes 50% at Birchwood Nursing Home and Rehabilitation, and 50% at Shep 3 North, alternating every two weeks. At the nursing home and rehabilitation, the Nurse Practitioner will provide continuity of care and assist in discharge planning.

Shep 3 is a dedicated service for patients that are good candidates for discharge to nursing home and subacute facilities, that with dedicated support will be more likely to make this transition. Overarching principal will be for multi-disciplinary team (providers, nursing, case management) to work closely to ensure success of long-term placement. Unit capacity 20-22 beds.

This is a bargaining union position.
