Responsibilities

About the Position:

During a visit that can last up to one hour, Matrix providers review and observe a member's current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member's overall health and well-being that can be difficult to capture during routine office visits.

The Matrix Comprehensive Health Assessment helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit.

Location: Home, SNF, Telehealth and other community environments in Santa Rosa, California 95409

Hours: Days, Evening, Nights, weekend.

Flexible Scheduling

PAY: $115.00 per completed Assessment

Matrix Provider

  • What to Expect:
  • Make a difference in people's lives by conducting Pediatric/Adult/Geriatric Assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols.

  • Work collaboratively with physicians, case managers, social workers, family members, key caregivers and any appropriate ancillary medical personnel as appropriate. Collaborate with Primary Care Physician (PCP) on patient education. Provide follow-up.

  • A nationwide network of advanced practice providers to build and maintain relationships.

  • Services may be provided in a variety of venues to include; Home Visits, Skilled Facility Visits, Mobile Clinic Visits, Telehealth Visits, and Retail Clinic Visits.

Qualifications

Must Haves:

  • Master's Degree required OR commensurate experience and satisfactory completion of NP licensure.

  • Current RN and NP licensure in state of practice to include prescription authority or the ability to obtainprescriptive authority.

  • Board certified by the AANP, ANCC or the AACCN in a Matrix approved specialty.

  • Have a current BLS, ACLS or CPR certification.

  • Have a Minimum of 1 year experience as a Nurse Practitioner

  • Have strong computer skills and familiarity with Employee health/medical record software.

  • Possess excellent verbal and written communication skills with patient, clients and colleagues.

  • Be comfortable and flexible with frequent change.

Travel Requirements:

  • Have the ability to travel, a valid state driver license and able to drive a car, proof of adequate automobile insurance coverage for the state of residence.

  • Travel may be required

The program has been in operation since 2013 directed by a Geriatrician. This opportunity will allow you to work and collaborate with a Geriatric trained physician and Medical Director, Dr. Dan Osterweil. Dr. Osterweil, MD, FACP, leads the senior-focused HealthCHEC Comprehensive Health Evaluation operations. He completed his geriatrics fellowship at UCLA. His areas of expertise include cognitive and functional assessment, management of dementia and continuous quality improvement. About HealthCHEC- Comprehensive Health Evaluation Center HealthCHEC- Comprehensive Health Evaluation Center is an innovative member centered program were members of SCAN are invited to complete their Personal Health Assessments. The Personal Health Assessment includes reviewing and completing a detailed medical history, complete review of tests, screenings and preventative care the member has received, review of current medications and a complete physical exam. Upon completion of the Personal Health Assessment, our program develops a recommended care plan, discusses it with the member and sends a report to the member's Primary Care Physician. The job The Nurse Practitioner (Napa/Sonoma County) will provide evaluative assessments and care to SCAN members via home vists, clinic visits, and telehealth. Practice medicine within the scope of a CA Nurse Practitioner license. Provide clinical leadership in implementing, training, monitoring and improving Quality Hierarchical Chronic Condition initiatives. Organize and complete Chronic Care Assessments and chart audits for members. Will function as an independent healthcare provider with an advanced degree of knowledge, skill and expertise in history taking, physical exam, treatment and clinical care management of members. Collaborate with SCAN and Primary Care Physician's Case Management team, HCC Quality team, and Quality Improvement team to achieve corporate goals. You will * Perform Chronic Care Assessments for members; interpret and evaluate diagnostic tests to identify and assess individuals' clinical problems and health care needs. * Engage with providers and members to create a trusting environment; function in a collaborative relationship with physicians and other health care professionals in the clinical management of members; maintain a good working relationship with other personnel, medical staff and administration. * Provide health education to members; work with members in the prevention of illness, promotion of health, and management of member's adjustment to chronic health problems. * Conduct physical exams and assessments including diagnostic exams; take medical histories; diagnose and document member's health conditions; conduct wellness exams, administer vaccinations and draw blood for lab tests (if necessary). * Develop care plans in collaboration with members, case management and primary care physicians; educate and advise on medical issues as needed. * Consult with physicians and Medical Director as needed; ensure Primary Care Physicians are aware of member's health conditions and progress. * Provide referrals both internally and externally to appropriate medical resources. * Document member visit/interaction accurately and timely. * Develop an understanding of SCAN's encounter data submission process and CMS requirements; participate in the grievance, audit and continuing education, quality assurance, and quality improvement programs. * Serve as an on-going resource to providers and case management teams; serve on and participate in appropriate clinical committees, as assigned. * Report suspicious, unethical or illegal conduct and cooperate with approved investigations and inspections. * Attend to patients via home vists, clinic visits, and telehealth Requirements * Master's degree in Nursing * Current California Nurse Practitioner license in good standing * NPI and DEA license * Current First Aid/CPR/AED certification. * Excellent assessment skills required. * Two (2) years of experience working in a medical office or hospital setting. * Experince with telehealth strongly preferred * Understanding of ICD-10 Coding. * Ability to problem-solve and to prioritize changing member conditions required. * Knowledge of efficient interviewing practices and techniques used in order to gather information. * Strong interpersonal skills with the ability to build relationships with providers and members. * Ability to maintain member confidentiality required. * Knowledge of HIPAA requirements. * Knowledge of and/or ability to learn and use computer based applications such a MS Office and electronic medical record systems. * Ability to work on a team and maintain good working relationships with staff and members required. * Ability to remain calm and be resourceful in emergency situations required.
