Responsibilities

About the Position:

During a visit that can last up to one hour, Matrix providers review and observe a member's current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member's overall health and well-being that can be difficult to capture during routine office visits.

The Matrix Comprehensive Health Assessment helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit.

  • Nurse Practitioner

Location: Home, SNF, Telehealth and other community environments in Yuba City, CA

Hours: Days, Evening, Nights, weekend.

Flexible Scheduling

Salary: $110.00 per CHA per 40 assessments/month: $4,400.00

Matrix Provider

  • What to Expect:
  • Make a difference in people's lives by conducting Pediatric/Adult/Geriatric Assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols.

  • Work collaboratively with physicians, case managers, social workers, family members, key caregivers and any appropriate ancillary medical personnel as appropriate. Collaborate with Primary Care Physician (PCP) on patient education. Provide follow-up.

  • A nationwide network of advanced practice providers to build and maintain relationships.

  • Services may be provided in a variety of venues to include; Home Visits, Skilled Facility Visits, Mobile Clinic Visits, Telehealth Visits, and Retail Clinic Visits.

Qualifications

Must Haves:

  • Master's Degree required OR commensurate experience and satisfactory completion of NP licensure

  • Master's Degree required OR commensurate experience and satisfactory completion of PA licensure

  • Current RN and NP licensure in state of practice to include prescription authority or the ability to obtainprescriptive authority.

  • Board certified by the AANP, ANCC or the AACCN in a Matrix approved specialty.

  • Have a current BLS certified and ACLS preferred certification

  • Have a Minimum of 1 year experience as a Nurse Practitioner. Have the ability to travel either in state or out of state

  • Have strong computer skills and familiarity with EMR software

  • Possess excellent verbal and written communication skills with patient, clients and colleagues

  • Have a positive and outgoing personality, and be a team player. Be comfortable and flexible with frequent change

Travel Requirements:

  • Have the ability to travel, a valid state driver license and able to drive a car, proof of adequate automobile insurance coverage for the state of residence.

  • Travel may be required

This may necessitate tearing down and rebuilding whole sections of the store; the merchandiser must be able to follow a planogram, or diagram, of the new shelf or display. Resets can be quite large and complex, and completing them successfully requires merchandisers to be able to complete the work on schedule and according to plan. Responsibilities * Performs stores/tasks in efficient/cost effective manner * Accurately reports all completed retail tasks via the appropriate designated systems on the day the work is performed. * Communicates effectively with store personnel regarding tasks, sales activities, promotions, and client/sales plan objectives * Completes required training and certification programs. * Engage every work day with CROSSMARK'S communication tools for the purpose of accurately planning, reporting, and reviewing work. * Ability to implement retail schematics and merchandising materials as assigned. * Participate in team tasks onsite with clients. * Follows company policies, procedures, and position responsibilities. * Schematic implementation: compliance and shelf conditions * Implement customer approved schematics/Plan-O-Grams, tagging all items and obtaining management approval. * Must be able to take direction tagging, rotating and placing products on shelf. * Able to complete timely and accurate reporting (web based) of recaps, expense reports, timesheets, etc. This job description does not imply that the above functions are the only tasks that may be performed. Operating a Personal Vehicle: In the event the associate drives a personal vehicle for business purposes, the associate is required to maintain a valid state-issued driver's license and the minimum auto insurance coverage specified by the associate's state of residence.
