We have a new position for a Nurse Practitioner with Main Line HealthCare Primary Care Practice in Downingtown, PA

Our board-certified provider's offer care for patients age 18 and olderfor both well/preventative care visits, Medicare Annual Wellness visits, and for more urgent care when you are not feeling well.

Our areas of interest include preventative care, chronic disease management, and management of diabetes, high blood pressure, chronic obstructive pulmonary disease (COPD), asthma, headaches/migraines, dementia, thyroid disorders and Parkinson's disease. We perform many procedures including spirometry for asthma and COPD, suture removal, vaccinations, EKG, urine analysis, throat swabs, and influenza testing.

This is a Full Time, Days shift 40 hour/week position with possible 1 evening requirement per week and rotation treating patients in a Nursing Homes in Coatesville and/or Newtown Square, PA

Responsibilities to include:

The role of the CRNP is central to the unit based multidisciplinary team in leading quality and evidence based care initiatives that will result in achieving optimal patient outcomes and improved patient satisfaction. The CRNP extends and expands the role of the physicians, enhances continuity of quality patient care and amplifies communication among all professional care givers, the patient and his/her family members.


  • Completion of an accredited Nurse Practitioner program

  • Licensed by the Pennsylvania State Board of Nurse Examiners as a Certified Registered Nurse Practitioner (CRNP)

  • Participation in continuing education

  • BLS certification

  • ACLS certification

  • Certified Registered Nurse Practitioner in the State of Pennsylvania

  • Successful completion of the MLH credentialing process through the Medical Staff office, with temporary or permanent privileges approved


  • 2 years Nurse Practitioner experience preferred

  • A minimum of two years nursing experience in specific clinical area

  • Must have strong written and verbal skills of the English language

The RN is independent in his/her application of geriatric nursing knowledge and skills. He/she manages complex clinical situations for which he/she is responsible. Works with other nursing personnel in the delivery of care and services according to each participant's comprehensive participant assessment and plan of care. Assists in managing clinic in accord with facility philosophy, in compliance with standards of professional nursing practice, and with all state and federal laws and regulations. Utilizes resources in an efficient and economic manner to ensure that each participant achieves the highest achievable physical, mental and psycho-social well- being. ESSENTIAL DUTIES AND RESPONSIBILITIES ASSESSMENT: Collects participant data and completes clinical entry fields with appropriate responses according to unit standards; identifies participant's over problems/needs and sets priorities; identifies problems requiring further referral and/or follow up; interprets and records latest diagnostic results; performs advanced nursing using critical thinking skills. Planning Care: Develops a plan of care based on nursing process and which incorporates the plans of other disciplines and continuing care needs; includes the participant/family in developing or revising the plan of care; makes referrals to multidisciplinary support services. Initial and periodic assessments (minimally every 6 months) to be completed prior to the scheduled team meeting. Communicates participant changes to team members. Implementing /Managing Care: Care provided conforms to accepted practice standards; provides correct treatments/ procedures and other care as prescribed and according to participant care standards ; demonstrates understanding of age-related characteristics and needs of participants served; explains nursing procedures in appropriate forms; provides participant and family education in coordination of 24 hour care delivery; identifies emergency situations and independently initiates advanced therapy; acts as coordinator of participant care with other health care personnel and evaluates participant care measures instituted; understands and demonstrates respect for participant rights and utilizes established mechanisms for management of ethical issues in participant care. Analyzes data gathered from rounds and record reviews and acts promptly to resolve problems and to continuously improve quality. Participates in formal quality assessment and assurance activities as directed by the Quality Assurance Committee. Evaluation: Records participant care delivered as planned and any variation, with appropriate rationale; makes and records observations related to impending or associated problems; implements nursing measures related to impending or associated problems. Leadership: Using nursing process, assists with delegating nursing personnel, integrates cost effective measures into nursing practice; recognizes unit problems and takes responsibility for documenting same; demonstrates active participation in QI processes; complies with LIFE Program expectations to meet staffing demands based on participant care needs; complies with policies addressing safe working conditions and takes initiative when unsafe conditions appear; recognizes inappropriate and/ or ineffective participant care management and intercedes as appropriate, resolves issue/problem and completes written reports, fulfills call for unit as assigned and adjusts staffing for census (volume) and acuity. Participates in self-development and education activities, educates self and nursing peers by observing practice, interpreting facility policies and procedures, identifying learning needs and taking advantage of opportunities to teach on the unit. Fulfills state required continuing education requirements. Practices in manner consistent with infection control procedures. Consults with Nurse Practitioner and or Medical Director with any questions related to infection control. Ensures sufficient number of staff; evaluates staffing based on LIFE daily census, participant acuity, and other participant needs. Adjusts staffing pattern accordingly. Communicates difficulty meeting participant needs and maintaining regulatory compliance promptly to Director of Clinical Services. Completes required documentation of care and services delivered including subjective findings, objective symptoms, interventions and participant responses to interventions. Completes required documentation of special circumstances including accident/incident reports compliance with state and federal laws and regulations. Reports relevant information to the Medical Director, Center Director, QA Coordinator and interdisciplinary team re: functioning and communication, regulatory compliance issues, participant risk factors, sudden changes in participant behavior, expressed dissatisfaction by participant or family, reviews of treatment unexplained injuries, staff behavior (positive and negative), staffing needs, equipment and supply needs, opportunities to revise policies, procedures, improve quality. Reports all injuries to self or others to supervisor immediately. Participates in fire and disaster drills. In the event of an emergency carries out assigned duties to assure participant safety.
