Hennepin Healthcare has a current opening for a Nurse Practitioner or Physician Assistant within our Cardiology Clinic. This is a 1.0 position (day hours Monday - Friday only, no afterhours or weekend duties) and a primary ambulatory role in general cardiology and electrophysiology. RESPONSIBILITIES
Provide direct primary cardiology care and acute care services to adult patients as part of the primary care interdisciplinary team in the ambulatory settings
Perform initial and ongoing assessment of patients' medical physical and psychosocial status
Obtain and evaluate health and medical history, perform physical exams, conduct preventive screening procedures based on age and history. Identify medical and health risks
Complete accurate and timely documentation of services, interventions, plan of care, referrals, and follow-up
Formulate appropriate differential diagnosis based on history, physical examination, and diagnostic testing
Identify needs of the individual, family or community and develop a plan of care and/or education plan
Order/conduct/interpret diagnostic tests, therapeutic treatments
May prescribe, administer and dispense therapeutic devices and drugs
Make appropriate referrals to other health professionals and community agencies
Reassess and modify the plan as necessary to achieve medical and health goals
Develop a patient education plan relevant to the condition and patient/family needs
Demonstrate respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment
Work in partnership with electrophysiologist to facilitate patient care and execute care plans
QUALIFICATIONS
Minimum Qualifications:
Master's degree in Nursing or Physician Assistant (PA) studies from an accredited program
Knowledge of electronic medical documentation system/software (e.g. Epic)
Current licensure to practice as an Advanced Practice Registered Nurse (APRN) or PA issued by the State of Minnesota (or eligibility) · Alignment with the mission of Hennepin Healthcare
