This role is within our Patient Connect program. Optum Complex Care Management provides the Patient Connect Program for United Healthcare members in a short stay/transitional setting with focus on reduction in 30-day hospital readmissions, improved completeness and coding accuracy of diagnosis and medical record documentation, increased closure of STAR/HEDIS quality measures, improved completion rates of Advanced Care Planning Directives, and improved patient and family satisfaction and discharge and post discharge support. The Optum practitioner will provide a one time comprehensive health assessment for the member in collaboration with the case management team to support better care coordination and health outcomes. This is an autonomous role that creates enormous satisfaction for the NP as you impact the care and support of our population.

Primary Responsibilities:

  • Work with primary care physicians to provide the best care possible

  • Collaborate with the nursing staff and the patients' families

  • Collaboration with case management team

  • Documentation, coding and gap closure

In addition, you may be asked to help with our ISNP program which is providing primary care to patients in long term care settings.

Required Qualifications:

  • Current, unrestricted NP license in the state of assignment or the ability to obtain

  • Ability to obtain DEA licensure/Prescriptive Authority post-hire in states where applicable

  • Nurse Practitioner national certification as ANP, FNP, or GNP required or the ability to obtain

  • Clinical background in adult, family or geriatric specialties

  • Experience working in a nursing home or with seniors in other settings

  • Intermediate level of proficiency in PC based word processing including Microsoft Word and Outlook

  • Weekday and weekend availability

Preferred Qualifications:

  • Experience in short term care/rehab
  • 1+ years of hands on post grad experience within Long Term Care

The Agawam Counseling Center (ACC) of Behavioral Health Network is currently seeking Outpatient Clinicians to work Full time in Agawam, MA. NOTICE In accordance with the BHN Mandatory COVID vaccine policy - all staff, interns, temps, residents and contractors are required to be fully vaccinated no later than day one of employment. Medical or religious exemptions will be offered and must be approved prior to starting work. The same guidelines noted above apply during flu season (defined as December 1st - through end of flu season as announced by the CDC) for BHN's Mandatory Influenza Vaccine policy. Clinicians at ACC provide individual, family, couples, and/or group treatment based on clinical theories and principals.
