Nurse Practitioner NP or Physician Assistant PA Part Time Position. Set your own schedule and earn an excellent Income!
This is an autonomous field based, prevention/education focused opportunity that takes you out of the mundane office setting.
Our healthcare providers conduct comprehensive, in-home Health/Annual Wellness Visits with Medicare patients as part of their health insurance benefits. The evaluations are conducted in a face-to-face manner where the member resides.
Within the visit the clinician reinforces the importance of staying current on covered preventive services while identifying potential gaps in care, which stems unnecessary hospitalizations and ER visits.
Nurse Practitioners NP or Physician Assistant PA needed to conduct In-Home Health Assessments in Hemet/San Jacinto, CA area Part time.
Our Providers enjoy:
Flexibility - Design your own schedule with weekend visits as an option (10 assessments per week for part time status)
No prescribing or treatment, simply performing consultative assessments/evaluations
Fixed payment per completed appointment; each visit takes approximately 60 minutes
Must be State licensed and Board Certified Nurse Practitioner or Physician Assistant with NPI
The following will be provided:

Credentialing services, paid training.
Malpractice insurance
Fixed payment per visit
Arrangement of appointments with Medicare members within a certain time frame
Web tracking and scheduling tools to maximize your travel time
iPad Provided
Mileage Reimbursement

You will use compassion, dedication, and your expertise to foster treatment team unity, build trust, and nurture rapport with our patients.</p> <p>If you are flexible, dedicated, and want to enhance your already top-notch skills, then consider working for us.<br />  </p> <p><strong>Requirements of the Psychiatric Nurse Practitioner (PMHNP):</strong></p> <ul> <li>You have a current, unrestricted state nurse practitioner (NP) license.</li> <li>You have completed an RN accredited program.</li> <li>You have completed an accredited program in Psychiatric/Mental Health Nurse Practitioner (PMHNP).</li> <li>You have a minimum of two (2) years' clinical experience, home-based care preferred.</li> </ul> <p><strong>Responsibilities of the Psychiatric Nurse Practitioner (PMHNP):</strong></p> <ul> <li>Perform clinical visits and oversight for patients with diagnosed or suspected behavioral/mental health conditions.</li> <li>Complete an initial detailed assessment on all newly-enrolled patients.</li> <li>Establish a patient management care plan upon enrollment and update it as needed. Meet with patient and/or their legal representatives and patient’s primary care physician (PCP) to review the care plan.</li> <li>Communicate directly with patient’s PCP to discuss changes in condition or medication.</li> <li>Build and maintain productive relationships with patients and their legal representatives, facility staff, PCPs, psychiatrists and other collaborating mental health providers to maximize continuity and adherence to the patient care plan.</li> <li>Participate in weekly meetings with interdisciplinary team to ensure behavioral health issues are addressed for high risk patients.</li> <li>Report any issues with complex mental health patients, severely mentally ill and active inpatient psych admission with the Chief Psychiatric Officer.</li> <li>Comply with long-term care policies and procedures in all facilities.</li> <li>Manage the efficient use of the in-office and field resources related to patient care and client services to meet office budget</li> <li>Complete other assignments as requested and assigned.</li> </ul> <p>This is a superb opportunity to join one of Modern Healthcare's Best Places to Work 2017.
