The Center for Digestive Diseases located at MedStar Franklin Square Medical Center provides superior care to our patients.# Our multidisciplinary team of experts is comprised of specialty-trained physicians, advanced practice clinicians, technicians and clinical coordinators.# Amongst this team, are four highly-trained expert colorectal surgeons who are seeking a full time Physician Assistant or Nurse Practitioner to join their team.# MedStar Health#s colorectal surgeons specialize in the diagnosis and treatment of surgical disorders involving the small intestine, colon, rectum, and anus, including colorectal cancers, inflammatory bowel diseases like Crohn#s Disease and ulcerative colitis, as well as other more common benign diseases like complicated diverticulitis, constipation and rectal prolapse. In addition, they are experts in anorectal disorders such as hemorrhoids, HPV-related anorectal diseases and bowel incontinence. Minimum Qualification- Education/Training: Graduation from a Nurse Practitioner program approved by the Maryland State#Board Examiners of Nurses. Experience: 2 years experience as a nurse practitioner in an outpatient clinical setting preferred. License/Cert/Reg: Registration with the Maryland State Board Examiners of Nurses. Primary Duties and Responsibilities Contributes to the achievement of established department goals and objectives and adheres to#department policies, procedures, quality standards, and safety standards. Complies with#governmental and accreditation regulations. Coordination/assisting inpatients with their disposition/discharge planning.# Establishes medical diagnoses and provides age-appropriate comprehensive and/or episodic care to#patients presenting with acute and/or chronic medical/surgical problems, following practice and/or#hospital policies and procedures. Orders and interprets diagnostic and therapeutic tests relative to patient#s age-specific needs,#effectively communicating positive findings to patient and referring physicians to ensure continuity of#patient care. Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities monitoring#effectiveness of interventions. Performs clinical procedures independently within scope of practice after demonstrating proficiency#in procedural skills pertinent to practice/specialty area. Performs history and physical assessments of patients that are accurate, complete, concise,#relevant and appropriate for age, gender, clinical problem and unit. Acts as a liaison between patient, physician, and allied health professionals to facilitate patient#s#transition between health care settings. Collaborates with multidisciplinary team members by making appropriate referrals. Monitors medical/emotional condition of patients, reporting changes to appropriate personnel. Facilitates staff, patient and family decision making by providing educational tools. Maintains accurate, complete, concise and timely documentation in the electronic medical record. Maintains a safe, comfortable, and therapeutic environment for patients and families in accordance#with hospital standards. Prepares, completes and maintains records of patient charges. Maintains all required reports, records, statistics, etc. Complies with governmental and accreditation regulations. Demonstrates behavior consistent with MedStar Health mission, vision, goals, objectives and patient#care philosophy. Participates in meetings and on committees and represents the department and hospital in#community outreach efforts. Participates in multi-disciplinary quality and service improvement teams.   • Job Summary

  • Performs higher level administrative and clinical duties under the direct on-site supervision of the Practice Manager and Medical Director in the ambulatory setting. Oversees the training and competencies of the CMA staff.

  • Minimum Qualifications

  • Education/Training

  • High school graduation or equivalent.

  • Experience

  • 5 years CMA experience or 1 year in CMA supervisory role. 2 years in medical office or ambulatory health environment.

  • License/Certification/Registration

  • CMA (Certified Medical Assistant) through American Association of Medical Assistants, or RMA (Registered Medical Assistant) through American Medical Technologists, or certification/registration from a nationally recognized agency. CPR certification.

  • Knowledge, Skills & Abilities

  • Clinical competency as verified annually. Advanced computer skills required. Advanced training as a health coach. Ongoing leadership skills training required.

  • Primary Duties and Responsibilities

  • Acts as a health coach to patients to assess readiness and ability to overcome health obstacles.

  • Conducts staff meetings and regularly distributes and ensures receipt of information regarding new developments, policies and procedures.

  • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations.

  • Cross-trained/serves as backup for health team members including: registrars, HIM, Referral Coordinators, Coders, Care Coordinators and Master MA as required.

  • Implements, monitors, and reinforces MedStar policies and procedures. Recommends new or modified policies and procedures to reduce cost and/or improve staff performance. Works with the Practice Manager to create an efficient practice.

  • Mentors staff by aligning with the mission and values of the organization. Fosters staff development.

  • Monitors logs to assure quality control.

  • Participates in multi-disciplinary quality and service improvement initiatives.

  • Performs all technical duties and responsibilities of a medical assistant.

  • Performs other duties as assigned.

  • Provides adequate training of new MA employees and conducts competency testing for all medical assistants. Ensures that certifications are kept current.

  • Provides population management through the use of outreach calls to proactively reach out to patients through risk stratification tools.

  • With direction from the Practice Manager, assists with MA employee recruitment and retention, employee relations, performance management, performance evaluation, and all other personnel actions for their staff. Actively works to create a cohesive and productive work team.

Our team collaborates with the patients' primary physician in the CCM program * Palliative Care: health plans look to us to provide community based palliative care in the patient's homes. We provide physical, emotional and spiritual support for patients who are facing life-threatening illnesses, in order to provide relief from pain and other physical symptoms, psychosocial and spiritual care, and emotional support and improve quality of life. We are currently seeking nurse practitioners who are passionate about serious illness care to join our growing house calls practice to provide both chronic disease management and palliative care as we further extend our operations serving a variety of Medicare Advantage, Medicaid, and Commercial clients. * Perform home visits with patients enrolled in Alegis Care's Chronic Care Management Advanced Care program (ACP). * Perform home visits with patients enrolled in Alegis Care's Chronic Care Management program (CCM). Initial visits are comprehensive evaluations with subsequent visits targeting risk factors for disease exacerbations and hospitalizations. * Working closely with other team members to ensure quality care is delivered. * Coordinate with patient's other providers including specialists and health plan case managers to ensure all are working together with the patients towards the same goals. * Working closely with members of our care team to ensure quality care is delivered in a timely manner. The care center will assist with scheduling and triaging of urgent issues. * Sharing after hour telephone on-call coverage with other licensed providers on the team. * Working with field and office RN case managers and medical assistants * Clinical documentation of acute and chronic healthcare issues, symptoms, and goals of care through patient encounter in EMR system * Documentation must be appropriate and thorough, and completed in a timely manner * Provider will ensure all appropriate consent forms are signed and dated * Provider will have frequent interaction with patients, coders, schedulers, and supervising physician * The provider will educate patients and/or patient's family on chronic medical conditions, preventative care, and medication adherence compliance Knowledge, Skills & Abilities: * Graduated from an Advanced Nurse Practitioner program in Adult, Geriatric, or Family Nurse Practitioner. * Prior clinical experience in palliative care, end of life, hospice, oncology, ICU, geriatrics is preferred. * Prior home based care experience is preferred * Prior clinical experience in geriatric, adult or family medicine is preferred * Experience with electronic medical records is required.
