SCHEDULE: FULL-TIME, MONDAY-FRIDAY

JOB SUMMARY:

The Certified Physician Extender (Nurse Practitioner) will work in collaboration with a physician to provide primary care services for ambulatory patients in the Jackson Clinic. Performs new patient, follow-up, and walk-in visits in collaboration with the primary care physician.

Documents and submits to the physician a list of patient encounters made each day for approval. Documents and submits charges for all patient encounters in the electronic medical record. Assists and takes initiative in handling issues of a non-routine nature. Performs other duties as requested.

MINIMUM QUALIFICATIONS REQUIRED:

Graduate from an accredited Nurse Practitioner or Physician Assistant program; National certification with the American Academy of Nurse Practitioners or the National Commission on Certification of Physician Assistants is also required. Previous work experience is required in the ambulatory care setting.

Current licensure as a CRNP or as a Physician Assistant in the State of Alabama. Strong oral and written communication skills required, as well as leadership and organizational abilities. We require an individual who is self-directed and who will accept accountability for their actions.

Primary Responsibilities: * Provide preventative and primary care for patients in assigned long term care facilities * Work with primary care physicians to provide the best care possible * Collaborate with the nursing staff and the patients' families * Work with the facility's utilization review process * Explore and learn more about palliative care Required Qualifications: * Current, unrestricted NP license in the state of assignment or the ability to obtain * 1 year of hands on post grad experience with Long Term Care * Ability to obtain DEA licensure / Prescriptive Authority post - hire in states where applicable * Nurse Practitioner National Certification as ANP, FNP, or GNP required or the ability to obtain * A clinical background in adult, family or geriatric specialties * Past experience working in a nursing home or with seniors in other settings * Intermediate level of proficiency in PC based word processing including Microsoft Word and Outlook * Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area Careers with Optum.
