Western Connecticut Medical Group is seeking a Nurse Practitioner or Physician Assistant (Patient Care Coordinator 3, APRN or PA) for our New Fairfield Family Practice office. It is a highly regarded well established practice with limited on call obligations. You will work with 3 Physicians and 2 Advance Practice Provider colleagues.

The APRN or PA provides medical care for patients in collaboration with a physician, assisting patients to recover from acute illnesses while minimizing the effects of chronic illnesses. Major accountabilities include, but are not limited to:
Examines, evaluates, diagnoses and treats patients to provide expeditious, high quality, customer focused care.
Orders the appropriate laboratory and diagnostic procedures, and prescribes medical therapeutics as required to evaluate the state of the patient's medical condition.
Interprets and integrates data to determine the appropriateness of the therapeutic treatment plan.
Consults with supervising physicians in accordance with the practice's policies and medical standards to assure standards of medical care and patient satisfaction.
Records all symptoms, objective findings, assessment, treatment plan and other information that is appropriate in the patient's chart and signs the record.
Instructs the patient and family regarding the patient's diagnosis, testing, expected outcome and future prevention.
Evaluates the outcome of the treatment plan by reviewing diagnostic results with the patient through phone calls and return visits.
Performs total quality improvement processes and initiatives to achieve the Practice's mission.
Responsible for following departmental policies and procedures to assure service with proper risk management.
Fulfills all compliance responsibilities related to the position.
Performs other duties as assigned.
This position requires a minimum formal education of Master Degree, State of CT RN and APRN or PA license, CT Controlled Substances and Federal DEA Registration
Willingness to work 8 hour shifts TBD between 8 am - 5 pm Monday through Friday.
Prior experience as APRN or PA preferred but new grads with some family medicine experience are welcome to apply.
