Texas Health Action is seeking a Nurse Practitioner or Physician Assistant at our Central Austin Kind Clinic to evaluate, plan, and provide care for patients in consultation with and under direct supervision of the attending Physician. This is a full-time, exempt position requiring 40 hours a week with some night and weekend hours.

The ideal candidate for the NP/PA role maintains effective communication with patients, staff and physicians, provides patient care in a competent, professional and courteous manner, and keeps abreast of current research and incorporates relevant findings into specific areas of clinical practice.

What you would do:

  • Performs clinical procedures under direction of attending Physician and according to approved protocols
  • Performs a wide range of activities, which include but not limited to: assessment, history & physical, recommendations for patient plan of care
  • Monitors patient progress
  • Maintains patient medical records according to established standards
  • Maintains communication with the supervisory physician(s) and attending physician to review patient records and care plans on a routine basis
  • Ability to collaborate with attending physicians, facility staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family
  • Ability to work independently-Interacts as a team member with all levels of clinical staff
  • Performs other duties as assigned

Requirements

Your previous work life and education must include:

  • Graduation from an accredited School of Nursing or Physician Assistants Program
  • 2+ years outpatient experience as a NP or PA
  • Unrestricted and valid license to practice as a Nurse Practitioner or Physician Assistant in the state of Texas
  • Current Drug Enforcement Agency (DEA) and Department of Public Safety (DPS) registrations IIIV for the purpose of writing prescriptions.
  • Current CPR certification
  • Working knowledge of HIPAA regulations

Your previous work life and education ideally would include:

  • 2+ years of experience in working with diverse populations, including LGBTQ+ and other marginalized populations
  • Masters Degree in Nursing
  • Working knowledge of Athena electronic medical record software
  • Working knowledge of healthcare insurance preferred
  • Bilingual: English and Spanish

The purpose of the collaboration is to provide a comprehensive, coordinated model of care to a mostly Medicare-aged population on a Medicare Advantage Plan. The NP or PA will educate providers on proper coding, risk adjustment metrics, capturing documentation, and adhering to quality protocols. The NP or PA will also interview patients to ensure proper documentation and to ensure all preventative screenings have been complete. The PSU will see patients in different clinics in the Austin area along with home, telemedicine, and mobile visits. The NP or PA performance will be tracked and comparatively reported using metrics related to the strategic direction and operational needs of the organization. Primary Responsibilities: * The NP or PA is expected to be able to provide care consistent with their licensure and/or certification * Diagnoses and treats both acute and chronic conditions such as diabetes, hypertension, kidney disease and other chronic medical conditions. The NP or PA will also treat acute conditions such as minor injuries and other acute medical conditions such as infections or exacerbations of chronic diseases * Orders and interprets diagnostic lab work, x-rays, EKG * Prescribes medications and other modalities of treatment as needed to treat those conditions listed above * Counsels patients on various disease conditions they may be treating to ensure that the patient is educated to their disease process, treatment, and expected outcomes * Works as an integral part of a comprehensive and collaborative team in order to provide quality evidenced-based care to our patients * Performs all other related duties as assigned You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.
