We are seeking a highly skilled and compassionate Advanced Practice Registered Nurse (APRN) to join our dedicated healthcare team at our Skilled Nursing Facility. The APRN will play a crucial role in providing high-quality, patient-centered care to residents, collaborating with interdisciplinary teams, and promoting overall well-being within the skilled nursing environment. DEEPARTMENT SCOPE OF SERVICE BMH Post Acute Care operates within the Brattleboro Area, providing essential medical services across multiple Skilled Nursing Facilities (SNFs). As part of the BMH-owned or affiliated medical practice, our department is committed to delivering comprehensive and high-quality care to residents within the SNFs. The practice is staffed by a team of physicians, physician assistants, and nurse practitioners, collectively working towards the common goal of enhancing patient outcomes and promoting overall well-being. POSITION SUMMARY:# Under the supervision of the infectious disease specialists, the nurse practitioner provides medical and nursing case management. Prescribes modalities to meet wellness and illness needs, including but not limited to medication, nutrition, exercise and traditional nursing treatments. Teaches and supports self-care activities including problem-solving and selection of treatment alternatives.


KEY RESPONSIBILITIES Clinical Assessment and Diagnosis:


Conduct comprehensive health assessments to determine the physical, emotional, and psychosocial needs of residents. #Formulate differential diagnoses and develop individualized care plans in collaboration with the healthcare team. #Monitor and evaluate the effectiveness of interventions and modify care plans accordingly.Advanced Nursing Care: Administer advanced nursing care, including medication management, wound care, and other treatments as needed. Provide skilled nursing services to residents with acute and chronic conditions. Collaborate with physicians and other healthcare professionals to optimize patient outcomes. Collaboration and Communication: Work collaboratively with the interdisciplinary healthcare team, including physicians, nurses, therapists, and social workers. Communicate effectively with residents, families, and caregivers to ensure understanding of care plans and address any concerns. Participate in regular team meetings and contribute to the development and implementation of care protocols. Education and Support: Provide education to residents and their families regarding health conditions, medications, and preventive care. Offer emotional support and counseling to residents and families during challenging times. Foster a culture of continuous learning by staying current with evidence-based practices and participating in professional development activities. Quality Improvement and Compliance: Contribute to ongoing quality improvement initiatives within the skilled nursing facility. Ensure compliance with regulatory standards, policies, and procedures. QUALIFICATIONS: EDUCATION Masters degree or Doctoral Degree from an accredited nurse practitioner program #Leading to certificate in Adult, Family or Pediatric practice. EXPERIENCE Two years of nursing experience prior to advanced degree preferred. LICENSURE/CERTIFICATION Current licensure as an APRN by the State of Vermont. #Certification as an adult, family, or pediatric nurse practitioner by a national organization, which is recognized by the Vermont State Board of Nursing preferred.
