Job Summary:

Provides help with all aspects of patient care, including diagnosis, treatments and consultations as part of a physician/nurse team. Works in both inpatient and outpatient situations and can perform independently or as part of a treatment team. Serves as an educator, case manager, consultant to staff and multidisciplinary health care team.

Job Description:

Principle Responsibilities

  • Provides clinical care for patients

  • Completes comprehensive patient assessments.

  • Determines diagnosis and treatment of illness.

  • Orders and interprets medical diagnostics in tandem with attending physician.

  • Prescribes medications, treatments, therapy and medical equipment.

  • Conducts patient education when working to manage chronic illness or wellness care.

  • Refers for specialty care when determined.

  • Consults with physician when patient needs outside of Nurse Practitioner scope of practice, diagnosis or treatment plan is unclear.

  • Participates in health care team

  • Promotes communication within the team and other interdisciplinary health care staff.

  • Acts as resource to members of the team.

  • Works with primary care team for program development.

  • Acts as liaison with outside agencies as needed.

  • Other duties as assigned.

Job Requirements

  • Master's degree Completion of accredited Nurse Practitioner program required

  • 2 to 5 years Minimum 2 years experience post Nurse Practitioner training strongly preferred preferred

  • Licensed Registered Nurse

  • MN Board of Nursing required upon hire and
  • Certified Nurse Practitioner (ANCC) - American Nurse Credentialing Center (ANCC) required certification within specialty upon hire and
  • BLS Tier 1 - Basic Life Support
  • Multisource required upon hire and
  • ACLS/BLS Tier 2-Advanced Cardiac Life Support
  • Multisource preferred
  • Drug Enforcement Administration upon hire required

Current, full, active, and unrestricted license, or otherwise recognized, as an Adult Nurse Practitioner in any State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. In cases of graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. Current, full, active certification as a Nurse Practitioner from the American Nurses Association or another nationally recognized certifying body. Nurse Practitioner Preferred Experience: Strong mentoring skills and experience working with a multidisciplinary team. Experience in implementing new program(s) within a large healthcare system. Experience in developing and implementing patient education content, based on the most current evidence-based practice. Demonstrates a high level of practice using evidence-based principles involving patients who have complex illnesses. Experienced with delivery of clinical services via various Telehealth modalities, including, but not limited to VA Video Connect and Clinical Video Telehealth. Experienced with other clinically based technology and virtual environments. Experience providing clinical care across facilities (interfacility coverage). Minimum of one two (2) years of experience in providing urgent care or related services via Telehealth modalities. Minimum of three (3) years of urgent care, emergency department, or related experience within the Department of Veterans Affairs. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
