Location: Elgin, IL & Geneva, IL

Status: Part-Time (potential to be full-time)

Summary

Looking for a personable and compassionate Nurse Practitioner who is willing to learn and works well on a team. Responsible for assisting the physicians in providing quality patient care.

ESSENTIAL DUTIES & RESPONSIBILITIES

The essential duties and responsibilities include the following. Other duties may be assigned.

• Perform office evaluation of new patients and determine treatment plans in conjunction with the supervising physician.

• Perform follow-up care for previous patients of the practice and treat appropriately.

• Perform pre-operative history & physical examinations for FVO patients.

• Collaborate with physician to continue the growth of the FVO Health Living Program.

• Collaborate with physicians to develop and maintain the FVO Bone Health Clinic.

• Prescribe legend and controlled substances.

• Order appropriate lab work and diagnostic testing. Monitor, review and interpret results of tests, and ensure the patient is informed of results in a timely manner, either by follow-up appointment in office or by phone call.

• Accurately and thoroughly document care given to patients.

• Complete hospital discharge summaries and admitting history and physicals.

• Bill patients according to established procedures.

Education & Experience Required:

• Graduated from an accredited Nurse Practitioner (NP) program.

• Licensed Nurse Practitioner (NP) in the State of Illinois.

• A minimum of three (3) years of experience, preferably in primary care/internal medicine.
