Job Description

Lutheran Medical Group is seeking an APP to join a large GI team in Fort Wayne! You will manage acute and chronic conditions in the clinic setting as well as assist with inpatient rounding responsibilities. Monday

  • Friday schedule with night triage call responsibilities in rotation. New or experienced APPs welcome!

Responsibilities and Requirements :

  • Manage acute and chronic patients in the OP clinic Monday
  • Friday
  • Conduct inpatient rounds and consults in support of physicians

  • Collaborate with team of physicians and APPs

  • Night call rotation is phone triage only.

  • Participate in interdisciplinary care planning activities

Location Description

Lutheran Hospital is a 396-bed tertiary care facility that opened on Thanksgiving Day in 1904. Lutheran's areas of specialized care include northern Indiana's only heart and kidney transplant programs, an accredited bariatric surgery program, Level II verified adult and pediatric trauma centers, an accredited and commended community hospital cancer care program and a certified primary stroke center.

In addition to transplantation, Lutheran's trailblazing history of heart care is evident through its accreditations as a heart failure institute and chest pain center.

It has also achieved advanced certification from The Joint Commission for ventricular assist devices and was the first hospital in northern Indiana to perform transcatheter aortic valve replacement. Lutheran Children's Hospital teams with 68 pediatricians from across the region and state who practice in 20 areas of pediatric specialty.
