We are currently seeking an experienced: HOSPICE ##PALLIATIVE CARE NURSE PRACTITIONER Full-time (Covering Asheville market) Job Description: Implements palliative care program requirements Completes medical visit with patients per guidelines and coordinate care with Palliative Care team as clinically indicated Conduct face to face encounter to provide re-certification information to Medical Director/Associate Medical Director for Hospice Patients Provide clinical findings of that face to face encounter Primary care physician, and/or supervising Medical Director/Associate Medical Director Communicate with patients/families at all levels. Communicate with all agency personnel Job Requirements: Master#s degree in Nursing required, with current licensure in the state of practice Certification as an Adult or Family Nurse Practitioner Experience in Hospice or Palliative Care nursing preferred Knowledge of the framework, function, and organization of a hospice care program. HOSPICE & PALLIATIVE CARE NURSE PRACTITIONER

Job Description:

  • Implements palliative care program requirements
