• Established team of 1 physician and 1 Advanced Practitioner

• Diabetes Self-Management program staffed with Certified Diabetes Educators (RN's and Dietitians)

• Healthy Living Program, a medically managed weight loss program supported by Dietitians and a Health Coach

• Experienced staff, both clinical and non-clinical, who know the patients and the community

• Full-time position, including dedicated administrative time

• 24/7 hospitalist program is responsible for adult inpatients

• Fully integrated system with clinics and hospitals on EPIC EMR to improve care coordination

Responsibilities

• Outpatient clinic only; Adult patients 18+
• Clinic hours Monday

• Friday 8AM-5PM

• No nights or weekends

• Shared call amongst the providers. Triaged through MyNurse

• Opportunity to work with patients with diabetes on an outpatient basis.

• Provide diabetes education and management to our patients and families

• Focus on planning, supervising treatments, and monitoring of diabetes related health concerns.

• Evaluate and treat multiple endocrine diseases.

• Work in collaboration with Diabetes Educators from an ADA Accredited Diabetes Self-Management program.

• Coordinating care of patients and working in collaboration with the patient's primary care provider.

• Exercises independent judgements in the assessment, coordination, management and evaluation of certain medical processes and procedures, under the guidelines of the scope of practice and medical protocols.

• Ability to work as a team member to improve the health of the people and communities we serve

Qualifications

• Experienced Endocrine Nurse Practitioner or Physician Assistant

• Board Certified

• Hold an unrestricted medical license in the state of Iowa

• Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practice

• Eligible to work in the United States
