Evaluates, examines, diagnose, and treats patients in collaboration with their supervising and or substitute supervising physician according to established guidelines and protocols. Maintains and promotes the well-being of the patient through preventative healthcare and guidance. Provides care in the outpatient setting only predominately Monday through Friday.

  • Regular, consistent, on-site, and timely attendance.

  • Ensures that the assessment of the patient and the direct and indirect patient care delivered is in accordance with appropriate licensing board, hospital policies, procedures, and protocols, and all other professional standards of care.

  • Collects patient assessment data in a systematic manner, focusing on the physical, psychosocial, age, cultural, and spiritual needs of the patient.

  • Performs systematic physical exam and history of present illness.

  • Documents assessment data, history of present illness, and physical findings on the appropriate medical record within established time frame.

  • Involves the patient/family and other healthcare providers when appropriate.

  • Manages the physical and psychological problems of the patient.

  • Identifies normal and abnormal findings.

  • Implements an individualized plan of treatment, reassess and revises plan as needed.

  • Evaluates the physical and emotional responses to medications and treatments.

  • Prescribes immunizations per recommended schedules.

  • Seeks appropriate consultation.

  • Develops an educational process for the provision and coordination of patient care activities that promote and maintain health, foster self-care and improves outcomes.

  • Ensures the process include education about safe and effective use of medications, pain and managing pain, available resources, obtaining further care and their responsibility for their care.

  • Acquires and distributes educational resources tailored to the patient's needs.

  • Provides appropriate instruction and guidance regarding immunizations.

  • Performs suture removal, and dressing changes as per supervising physician's directions.

  • Be available for call from home to assist in emergency surgeries.

  • Directs and provides appropriate Pre and Post-operative care.

  • Performs history and physicals as designated by the physician. Identifies normal and abnormal findings and reports to the physician in a timely manner.

  • Provides preoperative teaching to patients and or families.

  • Performs patient rounds ensuring accurate assessment, review of labs and diagnostic procedures, and reports overall progress to supervising physician.

  • Orders appropriate testing per established guidelines and as outlined by supervising physician.

  • Documentation supports a review of lab/diagnostic results, is reflective of all required history and physical assessment elements.

  • Communicates with supervising physician in a timely manner regarding changes in the patient's progress, status, or critical test results.

  • Prepares for and develops discharge planning in conjunction with supervising physician.

  • Dictates discharge summaries timely and appropriate.

  • Provides counseling and instruction to patients and or families as appropriate.

  • Performs clinical responsibilities in a manner that maximizes patient care and outcomes.

  • Assumes appropriate code team role during emergency situations and responds effectively.

  • Facilitates timely treatment of abnormal labs or other relevant findings.

  • Develops workflow patterns to facilitate patient transfers or discharges.

  • Participates in outcome management and Press Ganey scores.

  • In collaboration with the physician: Interviews, evaluates and assesses patients in the clinic setting. Provides recommendations for treatment.

  • Evaluates patient's current status providing recommendations for ongoing treatment. Assesses for alternate levels of care as necessary

  • Adjusts medication as indicated

  • Provides patient/family education regarding medication, rationale for use and possible side effects

  • Collaborates with the supervising physician and or substitute supervising physician and members of the treatment team to achieve desired outcomes and to assure continuous flow of patient care.

  • Alerts the treatment team to any significant changes in the patient's status

  • Within prescribed time frames reviews treatment goals and objectives with patients making modifications as appropriate

  • Communicates pertinent information timely and efficiently to the supervising or substitute supervising physician

  • Documents in the medical record.

  • Records patient's current mental status

  • Documents all screenings and assessments performed

  • Records medication changes, prescriptions written and samples provided

  • Documents patient's complaints, concerns and questions as appropriate

  • Documents any/all telephone contact with patients, families, pharmacists or physicians

  • Current licensure to practice as a Registered Nurse in the State of Pennsylvania

  • Current Basic Life Support (BLS) approved curriculum of the American Heart Association (CPR and AED) program.

  • Act 73

  • Current certification as a Certified Registered Nurse Practitioner in the specialty of Family Medicine.

  • Current licensure to practice as a Registered Nurse in the State of Pennsylvania

  • Current ACLS or completion within one year of hire/transfer date into department

  • Current Nurse Practitioner Licensure.

  • Related training/experience (preferably within the past five years)

  • Strong leadership ability, independent thinking and decision-making ability; analytical problem solving skills, sound judgment; excellent oral and written communication skills, must be able to function in a team environment.

  • Ability to plan, organize and prioritize multiple projects simultaneously. Interpersonal skills that facilitate positive, constructive interaction with personnel both intra and interdepartmentally. An ability to support an interdisciplinary team approach to patient care that crosses a comprehensive care continuum. Experience with JCAHO requirements.

Shifts: Rotation of shifts TECHNICAL STANDARDS Essential Functions: Defined as duties specific to the job, that, if removed, would change the entire purpose of the job; to include the Essential Physical Demands and Essential Working Conditions of this position. Core Essential Functions: * May be responsible for: Cold Food Preparation--Prepares food according to recipes and production sheets which meets the department's standards for quality, as measured by appearance, taste, temperature and portion sizes. * Follows recipes for regular and modified diets when preparing entrees, baked goods and vegetables to produce a consistent product. Reports any inconsistencies to the supervisor. Adjust recipes and substitutes ingredients if necessary based on diet modifications and yields. * Correctly and accurately produces or portions items on production sheets. Reports any discrepancies between menu, production sheet and recipes to the supervisor. * Maintains records for production area, which may include data about production and usage numbers, yields, and over-production/leftovers. Makes suggestions for use of leftovers. Plans production schedules with supervisor according to HACCP guidelines and menu requirements. * Follows proper cooking techniques, which include but are not limited to: weighing, mixing, measuring, cutting, and combining ingredients. * Performs simple mathematical calculations accurately when adjusting recipes, volumes, and weights of food products. * Cooks and prepares food using a variety of methods and HACCP guidelines to produce a quality product. * Operates/utilizes a variety of manual, electrical and/or gas cooking equipment and utensils as directed by recipes to produce a quality product. * Samples, inspects and tests food to ensure compliance with appropriate seasoning, quality, appearance, temperature and taste standards. * Suggests new menu items and contributes ideas, to improve the workflow. * Plans for the disposition of leftovers according to HACCP guidelines for time and temperatures of reheating and storage.
