Primary Responsibilities:

  • Conduct in - home assessments on health plan members, including past medical history, review of symptoms, physical examination, medication review and depression screening

  • Responsible for checking vitals, conducting a physical exam that includes monofilament test, urine dipstick and foot exam (as appropriate)

  • Identify diagnoses to be used in care management and active medical management in the furtherance of treatment

  • Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment

  • Communicate findings in your assessment that will be used to inform the PCP of potential gaps in care

  • Educate members on topics such as disease process, medication and compliance

  • Comply with all HIPAA regulations and maintain security of protected health information (PHI)

Since this position involves in - home visits, the ability to work autonomously and extensive travel will be required. You should also be comfortable working with geriatric patients who have a variety of health complications.

Required Qualifications:

  • Active, unencumbered license to practice nursing in the assigned state

  • Nurse Practitioner Certification from either the ANCC, AANP or AACN as Adult, Geriatric, Family NP

  • 1 year of clinical experience in highest level of education OR 3 years of experience as a Registered Nurse

  • Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area

  • Ability to obtain DEA licensure / prescriptive authority post - hire in states where applicable

  • Ability to spend at least 1 hour with a member in their home, which may be in understaffed or remote areas, in the presence of pets or individuals who are tobacco users

  • Ability to lift a 30 pound bag in and out of car and to navigate stairs and a variety of dwelling conditions and configurations

Preferred Qualifications:

  • Experience in Gerontology, Cardiology, Internal Medicine or Endocrinology

  • Home care or home visit experience

  • Excellent administrative and organizational skills and the ability to effectively communicate with seniors and their families

  • Computer literate and able to navigate the internet

Electric Operations is responsible for every aspect of PG&E's electric distribution and transmission operations, including planning, engineering, maintenance and construction, asset management, business planning, restoration and emergency response. The Estimating & Service Planning department supports the Work Order Fulfillment's execution of work. Estimating and Service Planning in the local headquarters is focused on our customer's requests for new services, rearrangement of facilities, and fielding support for the RMC estimating teams. The clerical team in the local headquarters provides administrative support to the construction and service planning teams along with the initiation of some of the centralized functions in the RMC clerical teams. Position Summary This job is represented by Engineers and Scientists of California (ESC) subject to collective bargaining. This position is charged with coordinating the installation of new gas and/or electric facilities, WRO projects, overhead and/or underground line extensions and services associated with large residential, commercial and agricultural projects. The position also handles street lighting, traffic controllers, un-metered load and State Highway lighting projects. Assigned SNBRs also act as coordinators for city and county relocation projects, street widening and street repaving that have conflicts with PG&E facilities. For training purposes, SNBR may handle no more than 3 IPE-level jobs at the same time and must work under the guidance of an IPE. The duties as outlined in this job description may vary by location. This position includes office work and site visits. Travel is estimated to be about 10% of the time with some overnight stays. You must have a valid driver's license and insurance coverage if using a personal car. This position includes office work and site visits. Job Responsibilities SAFETY: * Maximize public and employee safety by ensuring that job designs are performed to Company & State standards and meet all compliance requirements. Follow USP 22 guidelines. PROJECT MANAGEMENT: * Manages scope, schedule and cost for all assigned projects, from initiation to meter set. Scope and schedule is adjusted as appropriate to accommodate change (e.g., new customer needs, competition, and new regulation) * Coordinates all other departments or individuals who are involved with the project and works to define the project's background, objective, design scope, schedule, roles and responsibilities * Coordinates with others, such as Planning, Estimating, Construction and Metering, etc., in completing the project in a timely manner. Includes understanding of industrial project schedules and working closely with the work and resource group to prioritize work using available tools or technology, such as CLICK scheduling * Monitors project schedule and prioritizes work in coordination with the work and resource group * Schedules and conducts pre-engineering meetings, provides all pertinent data and addresses potential project risks including schedule setbacks and coordinates actions before setbacks occur * Conducts and documents pre-construction meeting * Ensures that project is performed as planned.
