Job Description:

TITLE: Psychiatric Mental Health Nurse Practitioner ("ACP")

DEPARTMENT: Elliot Professional Services Network, Inc. ("EPS")

REPORTS TO: Chief Medical Officer of Elliot Hospital and EMG Acute Care Services; and/or his/her designees.

PRIMARY PURPOSE

To provide comprehensive assessment, treatment and education to patients under the direction and supervision of a licensed physician. Coordinates treatment with other entities for the delivery of health care services. Provides care in a professional, efficient and ethical manner, consistent with the requirements of all applicable accreditation entities (DNV, Joint Commission, etc, as applicable), applicable laws and regulations of the State of New Hampshire, and all policies, rules and requirements of EPS and Elliot Health System ("EHS").

DUTIES AND RESPONSIBILITIES

Under the general direction of the Chief Medical Officer of Elliot Hospital and EMG Acute Care Services and Administrative Director, Behavioral Health and/or their designees and within established EPS and EHS and departmental policies and procedures, ACP shall perform the functions listed below. All duties shall be carried out: (i) in accordance with all applicable laws, rules and regulations, including those concerning protection of patient confidentiality; (ii) in a professional and competent manner and in accordance with the generally accepted standards of care; and (iii) in accordance with all policies, rules and requirements of EPS and/or EHS, and all bylaws, rules and regulations and policies of the medical staff of Elliot Hospital, as they may be amended from time to time.

Essential Functions:

Patient Assessment: Provide assessment and examination of individuals presenting to the Emergency Department with acute behavioral health complaints.

Amass information by interviewing the patient and/or family members to obtain an appropriate history, with regard to the patient's mental health, developmental status and behavior. Examines the patient to obtain objective physical data, performs appropriate systems review as indicated by the evaluation. Completes a mental assessment based on interview, examination and available data. Identifies medical and health issues and needs based upon the evaluation.

Treatment Planning: Based on the patient evaluation, makes appropriate treatment plans.

ACP shall at all times serve the best interests of the patient and shall exercise his or her best independent professional judgment when rendering care. Develops and implements a treatment plan, which may include diagnostic tests, pharmaceutical and non-pharmaceutical interventions, medical services as well as education. This may be part of a multidisciplinary team. ACP will be responsible for admitting patients to Elliot Hospital's Pathways unit under the supervision of a psychiatrist.

Documentation of Services: Maintains ongoing documentation of patient interventions.

All documentation must be completed promptly but no later than 48 hours after the date of service. This includes documentation of initial assessment, treatment plans, interventional and ancillary services and any other patient interactions. Documentation to be completed using an Electronic Medical Record as provided by Elliot Health System. Required to cooperate with any audits of patient charts conducted to ensure compliance with all coding and documentation requirements and any compliance education reasonably required by the Chief Medical Officer of Elliot Hospital and EMG Acute Care Services and Administrative Director, Behavioral Health and/or his/her designee.

Participates in Quality Improvement Activities: ACP will be required to attend and provide input in quality improvement and quality assurance projects as deemed necessary by the Chief Medical Officer of Elliot Hospital and EMG Acute Care Services and Administrative Director, Behavioral Health and/or his/her designee.

Such quality improvement activities will include activities related to enhancing and maintaining patient satisfaction. Participates in quality management activities that at a minimum comply with all applicable accreditation entities (DNV, Joint Commission, etc, as applicable), and the bylaws of the Medical Staff of the Elliot Hospital.

Patient Satisfaction: Demonstrates courtesy, respect and caring to patients and families.

Surveys will be distributed to patients at the close of their treatment. It is expected that the resulting information will be utilized to ensure that the ACP is within usual bounds when compared to colleagues within the specialty.

Participates in Practice Functions/Billing: ACP will be required to work with nursing, support personnel and administration in maintaining the efficient operation of the Emergency Department.

ACP will adhere to policies, procedures, standards of care, and standards of infection control applicable, as revised from time to time. Participates in staff meetings and cooperates with practitioners and co-workers to achieve departmental objectives. Reports concerns and problems as they arise to supervisor.

Demonstrates courtesy, respect and caring to colleagues and maintains professional rapport with external parties and organizations. Participates in staff meetings.

Collaboration: Collaborates with physicians, nursing, support personnel and administration in matters affecting patient care.

Participates in crafting policies, procedures, standards of care, standards of practice, standards of infection control, etc. Follows the processes and practices of EHS designed to meet the specific needs of the patients, e.g., access needs, educational needs, age-specific needs, medical surveillance, etc. Ensures the protection of patient rights and adherence to HIPAA regulations.

Education: Participates in planning, developing, implementation and evaluation of education programs, support and services offered by the EHS, which includes attendance at a reasonable number of corporate and Hospital meetings and education opportunities. EHS is continually expanding services and capabilities and is increasing its use of an electronic medical record, which episodically requires a significant time commitment by providers to develop proficiency and familiarity.

Scheduling: As a full-time ACP, provides an average of 38.5 clinical hours per week as scheduled by the Administrative Director, Behavioral Health.

The remaining 1.5 hours per week will be provided to EPS as administrative time. This time will be managed by the ACP to ensure all patient obligations are met, participation in various committees and practice meetings and coverage of a colleague for vacation, etc. ACPs with an agreed part-time schedule are prorated accordingly

Time off requests should be sent to Administrative Director, Behavioral Health or his/her designee for sign off and appropriate planning.

Credentialing: Submits credentialing and re-credentialing information to Medical Staff Services within one (1) week of receipt of application.

Location of Services: Services will be provided at the assigned practices as assigned by the Chief Medical Officer of Elliot Hospital and EMG Acute Care Services; Administrative Director, Behavioral Health and/or his/her designee.

Policies and Procedures: Adheres to all policies and procedures including but not limited to; infection control procedures, safety procedures, universal precautions, patient confidentiality and security policies. Ensures confidentiality of employee, legal, client/patient, budget and all company matters.

Space and Support Staff: Employer will provide ACP with appropriate and adequate support staff, space, and materials to meet his or her obligations under this Agreement.

Secondary Functions:

Participates in routine provider and management meetings to communicate issues and resolve problems.

Meets all requirements for State of New Hampshire licensure. Attains or renews board certifications.

Knowledge Skills and Abilities

Individuals must possess the knowledge, skills and abilities listed or be able to explain and demonstrate that the individual can perform the essential functions of the job, with or without reasonable accommodations, using some other combination of skills and abilities and to possess the necessary physical requirements, with or without the aid of mechanical devices to safely perform the essential functions of the job.

Ability to deliver patient care in a manner that is appropriate to patient's age, physical ability and intellectual development. Must demonstrate proficiency in assessing treatment and responses and adapting care to meet the needs of the patient population served.

Physical requirements include the ability to raise objects from a lower to a higher position or to move objects horizontally from position to position. The ability to extend hand(s) and arm(s) in any direction; pick, pinch, type or otherwise primarily work with fingers; perceiving attributes of objects such a size, shape, temperature or texture by touching with skin, particularly that of fingertips. Use upper extremities to exert force in order to draw, haul or tug objects in a sustained motion; to press against something with steady force in order to thrust forward, downward or outward.

Apply pressure to an object with the fingers and palm; ability to substantially move wrists; hands and/or fingers; bending body downward and forward by bending spine at the waist; bending body downward and forward by bending leg and spine; ability to bend legs at knee to come to rest on the knee or knees. Ability to express or exchange ideas by means of the spoken word. Essential functions include activities in which ACP must convey detailed or important spoken instructions to patients, ACPs, families and other employees.

Ability to receive detailed information through oral communication and to make fine discriminations in sound. The ACP is subject to normal inside environmental conditions, The ACP is not substantially exposed to adverse environmental conditions. Visual acuity sufficient for work which deals largely with preparing data and figures, accounting, transcription, computer terminal operation and extensive reading.

Requires the ability to plan and perform diversified duties requiring an extensive knowledge of a particular field and the use of a wide range of procedures, Involves the exercise of judgment in the analysis of facts and conditions regarding individual problems or transactions to determine what actions should be taken within the limits of standard practice. Ability to work in an environment where latex may be present.

OSHA RATING: 1

WORK SCHEDULE: Monday through Sunday, days, nights. Flexibility to accommodate workload as scheduled by the Administrative Director, Behavioral Health.

QUALIFICATIONS:

Education: Must hold a graduate degree earned in an accredited advanced practice registered nurse education program or have graduated before July 1, 2004 from an APRN education program accredited by a national accrediting body.

Experience: A minimum of 1 year experience as an advanced practice nurse is preferred.

Licensure/Certification: Must be currently certified by national certifying body for Psychiatric-Mental Health Nurse Practitioners (PMHNP- BC). Currently hold and maintain an RN and APRN license to practice in the state of New Hampshire or be eligible to obtain such license prior to date of hire.
