Currently looking for Board Certified Nurse Practitioners to conduct Health Risk Assessments on Medicare Advantage members. Must have experience in Family, Geriatric, Psychiatric, and/or Acute Care. Home care or hospice experience is a plus. Responsibilities of the Nurse Practitioner- Health Risk Assessments
  • Conduct comprehensive in-home health risk assessment to identify all active and chronic disease conditions as well as determine all physical, mental and social needs present at the time of the visit.
  • Obtain vital signs, measure BMI, review pharmacological therapy and conduct a physical examination.
  • Educate and counsel patient and family on any conditions identified during assessment and screening procedures.
  • Make recommendations and communicate findings to Primary Care Physicians through documentation of outcomes on provided tablet.
Requirements of the Nurse Practitioner- Health Risk Assessments
  • Master's degree in Nursing from an accredited school of Nursing.
  • Available to complete a minimum of 25 assessments per month.
  • Board Certification with current unrestricted Nurse Practitioner (NP) license.
  • Experience dealing with a complex set of patients with a relatively high level of acuity.

Demonstrate ICARE values and Service Pride Standards through actions.</p> <p> 2. Perform thorough and complete preanesthetic assessments and develops appropriate anesthesia plans.</p> <p> 3. Administer anesthesia in accordance with clinical privileges, including induction, maintenance and emergence.</p> <p>4. Select, obtain and administer anesthetics, adjuvant drugs, accessory drugs and fluids necessary to manage the anesthetic, to maintain the patient`s physiologic homeostasis, and to correct abnormal responses to the anesthesia or surgery</p> <p>5. Monitor, interpret and evaluate on going data as appropriate for the type of anesthetic and specific patient needs.</p> <p>6. Provide expertise in airway management both in and out of the surgical suite 7. Place and maintain epidural catheters as adjuncts to anesthesia.</p> <p> 8. Place and monitor arterial and central venous catheters including pulmonary artery catheters.</p> <p>9. Document pertinent information on the patient`s medical record.</p> <p>10. Respond to emergency situations by providing airway management, administration of emergency fluids or drugs, or using basic or advanced cardiac life support techniques.</p> <p>11. Act as investigators, collaborators, consultants, or assistants on research projects as needed.</p> <p>12. May provide clinical and didactic instruction to nurse anesthesia students with emphasis on the patient undergoing cardiac, thoracic and major vascular surgery.</p> <p>13. May evaluate and counsel nurse anesthesia students, making appropriate referrals when indicated.</p> <p>14. May assist in developing, implementing and evaluating didactic and clinical curricula for the nurse anesthesia students.</p> <p>15. May serve as a resource person to students and other health care professionals.</p> <p>16. May serve on a variety of institutional committees and participate as instructors in staff development and continuing education programs for both professional and non-professional staff members.</p> <p>17. Other duties as directed by the Chair of the Department, Sr. CRNA and other Anesthesiologists.<br />  </p> <p><strong>EDUCATION REQUIREMENTS</strong></p> <p>Bachelor of Science in Nursing and graduation from an accredited school of Nurse Anesthesia</p> <p>EXPERIENCE REQUIREMENTS 1-2 years of anesthesia experience preferred, will count residency experience.</p> <p>CERTIFICATES, LICENSES AND REGISTRATIONS REQUIRED Registered Nurse Licensure by the Texas Board of Nurse Examiners. Recognition as an Advanced Practice Nurse by the Texas Board of Nurse Examiners. Certification as a Nurse Anesthetist by the Council on Certification of Nurse Anesthetists and maintenance of that certification as required by the Council on Recertification of Nurse Anesthetists.<br />  </p> <p><strong>SPECIAL KNOWLEDGE, SKILLS AND ABILITIES REQUIRED</strong></p> <p>1. Effective interpersonal skills necessary to foster productive working relationships.</p> <p> 2. Effective oral and written communication skills.</p> <p>3. In depth knowledge and related skills of current anesthesia practices with emphasis on administration of anesthesia for the patient undergoing cardiac, vascular and thoracic surgery</p> <p>4. In depth knowledge of pharmacology with emphasis on anesthetic, cardiac and vasoactive drugs.</p> <p> 5. Thorough knowledge of anesthetic equipment.</p> <p> 6. Expertise in the placement of invasive monitoring devices including arterial, central venous and pulmonary artery catheters.</p> <p>7. Ability to move, manipulate, and safely operate anesthesia equipment during the administration of all types of anesthesia to all sizes of patients.</p> <p>8. Ability to adapt to work environment changes.</p> <p> 9. Ability to maintain confidentiality, including HIPPA regulations compliance.</p> <p>10.
