Position Overview:

Assesses, diagnoses, plans and implements the medical care of patients in collaboration with a physician. Functions within the full scope of licensure; accountable for the assessment, coordination delivery and evaluation of nursing care, including direct patient care, patient/family education and transitions of care.

Delivers holistic and individualized care to all patients in assigned area. Develops, implements, manages an optimal interdisciplinary plan of care that incorporates psychological, sociocultural, spiritual, economic, and life-style factors. Fosters and maintains collaborative relationships between patients, their family/support group, physicians, and other healthcare providers through timely and effective communications.

Adheres to hospital polices, industry standards, best practices, and applicable laws/regulations and codes to promote a quality, highly reliable patient experience. Engages in continuous growth and development in professional advanced nursing practice. Follows ambulatory care and inpatient standardized procedures, protocols, policy and procedures regulated by the State of California Nurse Practice Act.

The clinical Nurse Practitioner role requires advanced Nurse Practitioner knowledge of physical assessment and treatment, under the supervision of the HealthFirst Medical Director, with skills in chronic disease management, case management, and education. The Nurse Practitioner provides examination of and treatment recommendations to patients with chronic health conditions, including Type 2 Diabetes, Asthma, COPD, Hypertension, Hyperlipidemia, and others, as well as specialized nursing duties as directed.

The Nurse Practitioner plays an integral role in the patient care team, acting as a resource and educator to patients and their family members, as well as the HealthFirst Community Health Workers. Ongoing mentoring and teaching to non-licensed Community Health Workers goes hand in hand with daily patient care. The Nurse Practitioner will also review annual competencies for the Community Health Workers.

The Clinical Program Coordinator role requires the further development of an innovative chronic disease self-management program, oversight of the daily clinical schedule, and day-to-day oversight of clinic operations. This position is also responsible for interfacing with other hospital resources, implementing key strategic initiatives, building program volume, and developing relationships with the local community.

Responsibilities include leading a once-monthly HealthFirst team meeting of one hour, in addition to a weekly one-hour meeting with the clinic's Medical Director to discuss clinic operations. The end-of-year Report for the clinic which includes clinic data and advises on future plans is prepared by the Nurse Practitioner annually. Responsibilities also include planning of and attending the Community Advisory Board meeting, which occurs every 6 months. The Nurse Practitioner will use clinical experience to oversee and manage the program, as well as build a program responsive to the needs of the surrounding community.

EDUCATION:

Graduate of an accredited school of nursing.

Master's: Nursing

CERTIFICATION & LICENSURE:

NP-Nurse Practitioner (National certification)

NP-F - Nurse Practitioner Furnishing

RN-California Registered Nurse

BLS-Basic Life Support Healthcare Provider

Certification required based on the department requirements

TYPICAL EXPERIENCE:

5 years of recent relevant experience.

SKILLS AND KNOWLEDGE:

Demonstrated general knowledge and competence with regard to advanced nursing theories, concepts and practices; medical terminology; anatomy; and physiology.

Working knowledge of the healthcare industry, safety precaution policies, best practices regarding patient care and privacy, and changes in local/state/federal regulations.

Possess written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people.

Basic knowledge of computer applications, such as Microsoft Office Suite (Word and Outlook), Electronic Health Record.

Prioritize and delegate assignments and work within standardized policies, procedures, and scientific methods to achieve objectives and meet deadlines.

Work independently, as well as be part of the team, including accomplishing multiple tasks in an environment with interruptions.

Identify, evaluate and resolve standard problems by selecting appropriate solutions from established options.

Ensure the privacy of each patient's protected health information (PHI).

Build collaborate relationships with peers and other healthcare providers to achieve departmental and corporate objectives.

Pay Range is $84.58 to $122.63 / hour

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled.

Compensation takes into account several factors including but not limited to a candidate's experience, education, skills, licensure and certifications, department equity, training and organizational needs.
