We seek a Nurse Practitioner to join our Family Practice group in Elkton.

Working autonomously, the nurse practitioner (NP) conducts physical assessments, performs and interprets diagnostic tests, diagnoses and treats common conditions, provides screenings, interventions, counseling, and prescribes medications as needed to patients in specified age categories. The nurse practitioner is supported in their delivery of high-quality care with the use of an electronic medical record (EMR) system that guides and supports evidence-based practice throughout patient visits. Local collaborating physicians are continually available for consultations and mentorship. Specific duties include:

Providing accurate assessment, diagnosis, and treatment of common family illnesses for patients of all ages

Ensuring patient information, visit history, referrals, lab follow-ups, phone calls, and consults are carefully and completely documented in the EMR

Assisting patients with the management of their well-being with vaccinations and patient education

Identifying opportunities within the community for the development of outreach activities

RequirementsNurse Practitioner must meet the following requirements

  • Masters Degree level Family Nurse Practitioner program. A minimum of one year experience required.

  • Current National Board Certification

  • State of Employment license to practice as Advanced Practice Nurse

  • Effective verbal, written, and electronic communication skills

  • Outstanding organization skills and ability to multi-task

  • Initiative, creativity, problem-solving ability, adaptability, and flexibility

  • Ability to work without direct supervision and practice autonomously

  • Ability to collaborate with professional colleagues as necessary to provide quality care

  • Ability to consistently present a friendly, welcoming, and professional public-facing appearance.

VIEW JOBS 11/13/2018 12:00:00 AM 2019-02-11T00:00 * Position is full time weekends starting at 5pm Friday through Monday at 8am What You Will Do: As an on-call and triage nurse for Amedisys hospice you will respond to telephone requests for assistance and perform home visits for hospice and palliative care patients and families in their home setting after-hours and on weekends. * Answer and respond to incoming calls, referrals, and requests for assistance, completes admissions into care, and make follow-up visits on patients admitted the day prior; as necessary. * Communicate significant findings, problems, patient's needs, reports changes in condition or environment to the physician, clinical manager and other members of the hospice care team and obtains physician orders as required. * Maintain flow of communication between in-house staff, field staff and other departments, for coordination of activities and services. * Participate as a member of the interdisciplinary team in patient care conferences. Required: * Currentand unencumbered license to practice as a Registered Nurse. An employee is thisrole is only able to provide clinical guidance to patients in states in whichthe employee possesses a current and unencumbered license. * Nurse Licensure Compact (NLC) documents thefollowing: The mutual recognition model of nurse licensure allows a nurse tohave one license (in his or her state of residency) and to practice in otherstates (both physical and electronic), subject to each state's practice law andregulation. Nurses must legally reside in a NLC state to be eligible to have amulti-state license. See list of Nurse Licensure Compact (NLC) * Minimum of one (1) year of nursing experiencerequired. * CurrentCPR certification.
